Home My WebLink About940066.tiff RESOLUTION RE: APPROVE SEVEN PURCHASE OF SERVICES AGREEMENT AMENDMENTS BETWEEN HUMAN RESOURCES' AREA AGENCY ON AGING AND VARIOUS LOCAL AGENCIES AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with seven Purchase of Services Agreement Amendments between Weld County Human Resources' Area Agency on Aging and the following local agencies: 1. Meals on Wheels 2. Catholic Community Services Northern ($25,653) 3. Catholic Community Services Northern ($ 7, 931) 4. RVNA Home Care Services, Inc. 5. Eldergarden Adult Day Program 6. Charles J. Connell, Attorney at Law 7. Weld Mental Health Center, Inc. WHEREAS, said amendments commenced January 2, 1994, and will end December 31, 1994, with further terms and conditions being as stated in said amendments, and WHEREAS, after review, the Board deems it advisable to approve said amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the seven Purchase of Services Agreement Amendments between Weld County Human Resources' Area Agency on Aging and the abovementioned local agencies be, and hereby are, approved. WITNESSETH: WHEREAS, the "AAA" desires to continue to purchase home delivered meals from "MOW", and WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of January 2, 1992, NOW THEREFORE, it is mutually agreed: 1. The "AAA" will reimburse "MOW" $28,378.00 for 25,400 home delivered meals for 88 persons. 2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. 3. WITNESSETH: WHEREAS, the "AAA" desires to continue to purchase Hispanic elderly outreach services from "CCSN", and WHEREAS, the "AAA" and "CCSN" are desirous of amending the original agreement of January 2, 1992, NOW THEREFORE, it is mutually agreed: 1. The "AAA" will reimburse "CCSN" $25,653.00 for 3,700 units of outreach to 300 isolated and homebound Hispanic elderly. 2.
