RE: APPROVE AMENDMENT NO. V TO MEDICAL PLAN DOCUMENT OF WELD
COUNTY AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment No. V to
the Medical Plan Document of Weld County, and
WHEREAS, after review, the Board deems it advisable to
approve said Amendment, a copy being attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. V to
the Medical Plan Document of Weld County be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Amendment.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 15th day of
August, A.D. , 1990/.

to the Medical Plan Document of
Weld County
The Medical Plan Document of Weld County Employee Benefit Plan is hereby amended,
effective September 1, 1990, as follows:
The following services and supplies are added to MAJOR MEDICAL BENEFIT Number D -
COVERED MEDICAL EXPENSES:
- Charges for covered transplant donor fees are covered only if the donor
is a Weld County medical plan participant.
- Charges in connection with eating disorders
- Charges in connection with sleep disorders
- Medically necessary charges for amniocentesis and ultrasounds
- Charges in connection with the treatment of morbid obesity
- Occupational therapy
The following services and supplies are added to GENERAL LIMITATIONS:
40. any charges resulting from complications of a non-covered medical
condition or surgical procedure.
41. elective hospital admissions on Friday, Saturday or Sunday. If surgery
is scheduled for Monday, Sunday admission will be eligible for
consideration, if such admission is deemed medically necessary.
42. treatment for infertility.
43, personal comfort items while hospitalized, such as telephone,
television, etc.
44. charges in connection with the insertion and maintenance of an
artificial heart.
45. charges in connection with sex change surgery or counseling for gender
identification dysfunction.
46. charges for kerato-refractive eye surgery.

47. massage therapy or rolfing.
48. adoption expenses.
49. surrogate expenses.
50. charges for biofeedback or hypnosis.
51. genetic counseling.
52. charges for wigs or hairpieces.
53. any charges related to treatment or services rendered outside the United
States or its territories except while traveling for business or
pleasure.
54. charges for marital or family counseling.
55. charges for hair analysis. .
56. charges for or related to colonic therapy or colonic irrigation.
57. occupational therapy supplies.
IT IS AGREED BY Weld County that the provisions contained in the Plan Document
and Amendment No. V thereto are acceptable and will be the basis for the
administration of said Employer's Medical Benefit Program described herein.
