RE: APPROVE LETTER OF AGREEMENT WITH COLORADO DEPARTMENT OF HEALTH FOR STATE
PER CAPITA FUNDING AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Letter of Agreement between
the Colorado Department of Health and the Weld County Health Department for State
Per Capita Funding, commencing July 1, 1992, and ending June 30, 1993, with the
further terms and conditions being as stated in said letter of agreement, and
WHEREAS, after review, the Board deems it advisable to approve said letter
of agreement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Letter of Agreement between the Colorado Department
of Health and the Weld County Health Department for State Per Capita Funding be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said letter of agreement.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 5th day of October, A.D. , 1992, nunc pro
tunc July 1, 1992.

Dear Randy Gordon:
Your department's allocation of the State FY 92-93 Local Health
appropriation is $158, 191. With Contract #930242 you received
$139, 669, the part of the allocation that is from the state general
fund. Dan Gossert has notified you that you will also be eligible
for $18, 522 that must be used according to Federal MCH block grant
requirements.
You are also eligible to receive $3 , 568.60 for you work with child
care centers. This money is now available and can be forwarded to
you when we receive you signed copy of this letter. These funds
may be used only for enhancement of the child care environmental
health inspection and licensing program.
Finally, please submit your regional services proposal as soon as
feasible so the Board of Health can approve ' and we ca reimburse

Subjeet:State Per Capita Funding Agreement !
Enclosed for Board approval is a letter of agreement between the Weld County
Health Department and the Colorado Department of Health. The letter specifies
that the Health Department will receive $18,522 in federal MCH block grant funds
and $3,568 for completing a survey of Weld County child care centers.
The term of the agreement shall be from July 1, 1992 through June 30, 1993.
