HomeMy WebLinkAbout920834.tiff RESOLUTION RE: APPROVE NOTIFICATIONS OF FINANCIAL ASSISTANCE AWARDS FOR PLACEMENT ALTERNATIVES COMMISSION (PAC) FUNDS FOR NORTH COLORADO PSYCHCARE AND WELD MENTAL HEALTH AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Notifications of Financial Assistance Awards for Placement Alternatives Commission (PAC) Funds for North Colorado Psychcare and Weld Mental Health, comencing September 1, 1992, and ending May 31, 1993, with the further terms and conditions being as stated in said notifications, and WHEREAS, after review, the Board deems it advisable to approve said notifications, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the Notifications of Financial Assistance Awards for Placement Alternatives Commission (PAC) Funds for North Colorado Psychcare and Weld Mental Health be, and hereby are, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said notifications. Griego, Director Weld County Department o oci 1 Sere( ces SUBJECT: Notification of Financi 1 Asei tance" Awa ds for the Placement Alternatives Commission Funding DATE: August 28, 1992 Enclosed for Board approval are Notification of Financial Assistance Awards for North Colorado Psychcare and Weld Mental Health. The Placement Alternatives Commission (PAC) reviewed proposals under a Request for Proposal process and are recommending approval of two bids. These bids would be funded by PAC funds. 1. The total PAC funds available are $75,400 and would be awarded as follows: North Colorado Psychcare $38,678.65 (a unit cost of $16.56 an hour) Weld Mental Health $36,721.35 (a unit cost of $18.56 an hour) 2. The period of the award is September 1, 1992 through May 31, 1993. 3. North Colorado Psychcare would provide services to a monthly average of 1.5 adolescents (age 10 - 18) through its Youth Passages Program. Youth Passages offers 40 hours per week of treatment for up to six (6) weeks. The program utilizes milieu, individual, group, experiential, behavioral, and family therapy. When indicated, psychotropic medication is also administered. In addition, an accredited BOSES classroom staffed by an affective needs teacher addresses academic and behavioral issues in the classroom. 920 834 5-sob/Co George Kennedy August 28, 1992 Page two 4. The Family Preservation Program of Weld Mental Health would provide services to a monthly average of 4.4 families. The program provides multi-generational services for families to: a. stabilize current crisis situations and establish an operational crisis plan; b. develop coping skills and coping plans; and c. move into independent functioning.
