HomeMy WebLinkAbout920406.tiff RESOLUTION RE: REQUEST TO COLORADO DEPARTMENT OF SOCIAL SERVICES TO DESIGNATE WELD COUNTY AS A REGION FOR SINGLE ENTRY POINT PLAN AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has reviewed the Single Entry Point District Designation Criteria, marked as Exhibit A, as attached hereto and incorporated herein by this reference, and WHEREAS, after study and review, the Board deems it advisable to request that the Colorado Department of Social Services designate Weld County as a region for the Single Entry Point plan, and WHEREAS, the Board also deems it advisable not to begin providing long term care services under the Single Entry Point option at the initial start up date of July 1, 1993, but instead to continue operating Long Term Care Systems programs as they are currently delivered. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the request to the Colorado Department of Social Services to designate Weld County as a region for the Single Entry Point plan be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said request. R , -s � EXHIBIT A SINGLE ENTRY POINT (SEP) DISTRICT DESIGNATION CRITERIA: Single County: A county with 200 or more long term care clients (excluding nursing facility residents) can stand alone as a single entry point district. Eleven (11) counties could operate as single county single entry point districts or elect to be part of a multi-county district. The counties are: Adams, Arapahoe, Boulder, Denver, El Paso, Fremont, Jefferson, Lorimer, Mesa, Pueblo and Weld. Multi-County: All other counties must be part of a multi-county district with contiguous county boundaries and the district must have at least one full-time case manager. The district designation criteria are specified at 8.021.12 of the enclosed Department rules. By March 31, 1992: Enabling legislation (CRS 26-4-522) requires County Commissioners to select a single entry point district. A letter of recommendation from the County Commissioners is to be received at the State Department of Social Services by March 31, 1992, requesting single entry point district designation. All counties are to send a letter of recommendation even if the county can stand alone. The letter of recommendation must include the following information: a. Geographic boundaries of single entry point district. b. Organizational structure, operating methods and decision- making process of the board(s) of county commissioners. c. Assurances that the proposed district meets all criteria. d. Designation of contact person for the district. e. Resolution from county commissioners in each of the counties in the district supporting the district designation. By April 30, 1992: The State Department will approve and designate single entry point districts by April 30, 1992, if the district designation meets the single entry point district designation criteria. In the situation where a proposed district is adjacent to a county which is not included in another district and would potentially be appropriate for the district, the State Department will request consideration of including the county in the district. This is to advise the Department that the Board of Commissioners recommends that Weld County be designated a region for the Single Entry Point plan. The Board will continue to be open to amending this position if other regional options are acceptable. The Board also advises the State that Weld County has decided not to begin providing long term care services under the Single Entry Point option at the initial start up date of July 1, 1993. In the meantime, we will continue to operate our Long Term Care Systems programs as they are currently delivered. Box 1805 Greeley, Colorado 80632 Dear Ms. Piper: I am writing in regard to the recent designation of a long term care single entry point district for your area. Please accept my thanks on behalf of the State of Colorado for the thoughtful deliberations that everyone involved in your district designation process went through. I appreciate your cooperation in this effort to improve access to long term care services for elderly impaired persons and persons with disabilities in Colorado. An Evaluation Committee, composed of representatives of client advocacy groups and state staff, has reviewed the materials concerning designating a long term care single entry point district submitted by counties in your area. Your recommendation was reviewed against each of the individual criteria in the regulations as follows: A. All counties shall be included in the single entry point system; B. Counties composing a multi-county district shall be contiguous; C. A single county may be designated a district provided the county serves a monthly average of 200 or more non-institutional long term care clients; D. Multi-county districts shall not be required to serve a minimum number of clients; E. Each district shall have at least one full-time case manager employed by the single entry point agency that serves the district; and F. Each district shall assure adequate staffing by the single entry point agency to provide coverage for all case management functions and administrative support. Based on that review and the consequent recommendation of the committee, your district recommendation is hereby approved as recommended. The materials submitted from your district contained all of the required components: A. Geographic boundaries of the district; B. Organizational structure, operating methods and decision-making process of the boards of county commissioners; C. Assurance that the proposed district meets all of the above criteria; D. Designation of a contact person; and E. A resolution supporting the recommendation, passed by the county commissioners of each county in the district.
