Dear Monica,
Per our phone conversation on Tuesday October 11, 1994, I
am writing this letter to inform you of the proposed
additions to our corporate offices .
Please review the following proposals and determine if a
U. S.R. permit will be required.
Proposal I .
A 1, 500 sq. ft. addition onto the east
side of the current corporate west
building. This area is to be used for
office space.
Proposal II .
A 170 sq. ft. addition onto the north side
of the current corporate west building.
This area is to be used for records
storage.
Proposal III .
A new 800 sq. ft. frame construction
substance abuse building. This building
will be used to collect urinalysis samples
for testing, and provide an office for
counseling.
