RE: APPROVE WELD FOOD BANK COMMUNITY FOOD AND NUTRITION PROGRAM 1992
APPLICATION TO BE SUBMITTED TO COLORADO DEPARTMENT OF LOCAL AFFAIRS AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Community Food and Nutrition
Program 1992 Application for the Weld Food Bank to be submitted to the Colorado
Department of Local Affairs, with the terms and conditions being as stated in
said application, and
WHEREAS, after review, the Board deems it advisable to approve said
application, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Community Food
and Nutrition Program 1992 Application for the Weld Food Bank to be submitted to
the Colorado Department of Local Affairs be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said application.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 1st day of June, A.D. , 1992.

Applicant Program Description:
The Weld Food Bank, a not-for-profit 501-c-3 agency, accepts food donations
from various local producers and suppliers. Other sources of obtaining food
is food drives, Second Harvest National Food Bank Network, TEFAP, and purchasing
food.
In operating an Emergency Food Box Program for over six years we have noticed
some low income hungry persons do not make it through the system for various
reasons. We have and will continue to work with both the clients and system to
improve the situation. With the $4,260.00 we would purchase food and add any
extra perishable food items we have available to make "Mini" Boxes for these
persons. Included in these boxes would always be a protein item, dairy product,
soup, and macaroni and cheese dinner. When we have extra donations of garden
vegetables or the family has not received any commodities for the month those
foods would be added. All the coordination, eligibility criteria, etc. would
be decided by the Weld County Coordinated Food Box Committee of which Weld
County Department of Social Services is a member.
The goal is to feed hungry people and prevent waste of some spordic large perish-
able donations. The minimum number of persons served would be 1400.

GRANT AWARD
TERMS AND CONDITIONS
FY 1992
Section 681A(a) of the Community Services Block Grant Act, as
amended by Public Law 99-425, the Human Services Reauthorization
Act, and Public Law 501-101, the Augustus F. Hawkins Human
Services Reauthorization Act, authorizes funding for the
Community Food and Nutrition (CF&N) program.
The purposes of the program are to:
Improve coordination of existing private and public
assistance resources to better serve low-income
communities
Assist low-income communities to identify potential
sponsors of child nutrition programs and initiate
programs in unserved and under-served areas; and
Develop innovative approaches at the State and local
levels to meet nutritional needs of low-income
individuals.
This award is subject to the following terms and conditions:
1. Grants are awarded to States to be used for sub-grants with
statewide public or private, non-profit agencies. These
agencies must: demonstrate that their proposed activities
are statewide in scope; conduct activities which represent a
comprehensive and coordinated effort to alleviate hunger
within the State; involve a broad range of organizations
within the State also committed to alleviating hunger; and
preferably have demonstrated a track record successfully
implementing programs designed to alleviate hunger.
2 . Funds awarded by the State to public or private agencies
must support at least one of the designated purposes.
Activities funded through this award must also include
outreach and public education activities designed to inform
low-income and unemployed individuals of the nutrition
services available under various federally-assisted
programs.

3. There is no requirement for public or legislative hearings
concerning the intended use and distribution of these funds.
4 States are encouraged to provide funding for projects that
expand and enlarge other outreach activities or that
generate additional funds and resources from other sources
to support program purposes.
5. State must obligate these funds by September 30, 1992 .
6. Sub-grants to public and private agencies should be awarded
for a period not longer than twelve months.
7. Grant funds may not be used to defray State administrative
costs.
8 . Annual financial status reports (SF-269A) are required. The
financial status report is due 90 days after the end of the
Federal Fiscal Year, and shall constitute, unless otherwise
indicated, the final report. The State has up to fifteen
months beyond the end of the grant year to revise the final
report, if necessary. No unliquidated obligations can be
reported on the final report. The required financial report
must be submitted to the Administration for Children and
Families, Division of Formula, Entitlement, and Block
Grants, 370 L'Enfant Promenade, S.W. , Washington, D. C.
20447 .
9 . The expenditure of these funds is subject to the annual
audit requirements under the Single Audit Act of 1984
(Public Law 98-502) and Office of Management and Budget
Circulars A-128 and A-133 .
10. A final narrative report on the State' s CF&N program must be
submitted 90 days after the end of the Federal Fiscal-year.
The report must describe the sub-state recipient
organizations; the goals of the program; the purposes for
which the funds were expended; and the extent to which the
objectives of the program were achieved. The fiscal
narrative report must be submitted to the Administration for
Children and Families, Division of Formula, Entitlement and
Block Grants.

Enclosed for Board approval is a 1992 Application under the Community Food and
Nutrition Program. Our department is submitting this application on behalf of
the Weld Food Bank.
1. The application is for a period beginning July 1, 1992 through June 30,
1993.
2. The grant amount is $4,260 to serve 1400 individuals under the Emergency
Food Box Program.
