RE: APPROVE INTERDEPARTMENTAL AGREEMENT BETWEEN HUMAN RESOURCES' AREA AGENCY
ON AGING AND SOCIAL SERVICES AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Interdepartmental Agreement
between Weld County Division of Human Resources' Area Agency on Aging and the
Weld County Department of Social Services, commencing January 7, 1994, and ending
February 16, 1994, with further terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said
agreement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Interdepartmental Agreement between Weld County
Division of Human Resources' Area Agency on Aging and the Weld County Department
of Social Services be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 10th day of January, A.D. , 1994, nunc pro
tunc January 7, 1994.

INTERDEPARTMENTAL AGREEMENT
This agreement made and entered into, by and between WELD COUNTY HUMAN RESOURCES•
AREA AGENCY ON AGING, an agency of Weld County Government, hereinafter called
"WCAAA" and WELD COUNTY DEPARTMENT OF SOCIAL SERVICES, hereinafter called
"WCDSS".
WITNESSETH:
WHEREAS, WCAAA has been designated the Single Point of Entry for Long Term Care
Services for Weld County; and
WHEREAS, the WCDSS Home Care Allowance Program (HCA) will transfer to the Single
Entry Point to WCAAA on February 16, 1994; and
WHEREAS, WCDSS and WCAAA desire that the transfer of the HCA Program occur in an
efficient and effective manner;
NOW, THEREFORE, the parties do mutually agree as follows:
1. Nature of Agreement. WCAAA agrees to place, on loan, one full time
equivalent case manager at WCDSS to assist WCDSS Adult Services staff with
the preparation of the HCA caseload for transfer to WCAAA on February 16,
1994. The "loaned case manager" will report to WCDSS according to the
time scheduled by WCDSS.
2. Status of Employee. The case manager will be considered to be "on loan"
to WCDSS. Supervision of the case manager will be retained by WCAAA with
daily assignments directed by the WCDSS Social Services Administrator V
employee.
3. Training. WCDSS agrees to provide HCA orientation and training to WCAAA
staff including the WCAAA Single Entry Point Supervisor, the "loaned" case
manager, and WCAAA on site case managers who will need knowledge of the
HCA Program.
Training will include but not necessarily be limited to:
(a) Review of rules, regulations, and policies governing the HCA
Program
(b) Instruction on HCA intake, assessment, and redetermination
procedures
• (c) Training on access to the COIN data base system
(d) Training on fiscal and programmatic reporting requirements
5. Scope of Work. The "loaned" case manager will work with WCDSS adult
services staff to assist with intakes, assessments, redetermination, and
case file documentation. A final assessment report on the status of the
HCA case load will be written and submitted to WCDSS and WCAAA by February
16, 1994.

6. Coordination. The WCDSS Adult Programs supervisor and the WCAAA Single
Entry Point supervisor will meet immediately following the approval of
this agreement to develop a brief but specific transition plan which will
include but not be limited to:
(a) Weekly communication procedures between WCAAA and WCDSS
(b) Status report on HCA clients, addresses, and certification dates
(c) Ongoing training and support for the "loaned" case manager
7. Reimbursement. WCDSS agrees to reimburse WCAAA for all salary and fringe
for the one FTE case manager. (See Exhibit A) WCDSS also agrees to
reimburse directly to the "loaned case manager" $O.25 per mile for mileage
incurred by the case manager for HCA activity.
8. Term. Subject to the provisions contained herein, the term of Agreement
shall commence on January 7, 1994, and shall automatically terminate on
February 16, 1994.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement on
the day first written above.

Interdepartmental Agreement with Social Services
Subject:
Attached for Board approval is an interdepartmental agreement between the
Weld County Division of Human Resources' Area Agency on Aging and the Weld
County Department of Social Services.
With this agreement, the Area Agency on Aging will place a full time case
manager with Social Services for approximately ninety (90) days to assist
with the preparation of the Home Care Allowance (HCA) case files for
transfer to the Area Agency on Aging on February 16, 1994.
Weld County Social Services will reimburse Human Resources for the salary
and fringe benefits attributed to the "loaned case manager".
The HCA transfer is in conjunction with the designation of the Area Agency
on Aging as Single Point of Entry for Long Term Care Services for Weld
County.
