RE: APPROVE CERTIFICATION OF PROJECT COMPLETION FORM AND AUTHORIZE CHAIRMAN TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Certification of Project
Completion Form from the Colorado Department of Local Affairs, regarding
Community Development Block Grant Contract No. C859621, and
WHEREAS, after review, the Board deems it advisable to approve said form,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Certification of Project Completion Form be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said form.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 30th day of December, A.D. , 1991.

I hereby certify that all activities undertaken with funds provided under the above identified
Contract have, to the best of my knowledge, been carried out in accordance with all
provisions of the Contract. I also hereby confirm that the total amount of CDBG funds
expended-under the Contract is $ 199'1/99900 as previously reported in the final
Financial Status Report dated February 4 1991 , and hereby agree that the State
of Colorado has provided payments totalling this amount and is under no obligation to
make any additional payment under the Contract.
