RE: APPROVE AMENDMENT NO. IV TO MEDICAL PLAN DOCUMENT OF WELD
COUNTY AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS , the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS , the Board has been presented with Amendment No. IV
to the Medical Plan Document of Weld County concerning covered
transplant services, and
WHEREAS, after review, the Board deems it advisable to
approve said Amendment, a copy being attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. IV to
the Medical Plan Document of Weld County be, and hereby is ,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be , and
hereby is , authorized to sign said Amendment.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 18th day of
December, A.D. , 1989 .

AMENDMENT NO. IV
to the Medical Plan Document of
Weld County
The Medical Plan Document of Weld County Employee Benefit Plan is hereby amended,
effective January 1, 1990, as follows:
Page 21 - Major Medical Benefit
The following paragraph(s) are added in its entirety:
15. Charges for initial human organ and tissue transplants, limited to
kidney, cornea, bone marrow, heart. Covered transplant services are
limited to one per lifetime per Covered Person. Charges for subse-
quent transplants, whether for the same bodily organ or tissue or a
new bodily organ or tissue, are not covered.
Page 28 - General Limitations
37. Covered transplants are limited to kidney, cornea, heart, and bone
marrow transplants.
38. Any covered expenses for kidney, cornea, heart, or bone marrow trans-
plants are limited to one transplant per Covered Person per lifetime.
Charges for subsequent transplants, whether for the same bodily organ
or tissue or a new bodily organ or tissue, are not covered
IT IS AGREED BY WELD COUNTY that the provisions contained in the Plan Document
and Amendment No. IV thereto are acceptable and will be the basis for the administration
of said Employer's Medical Program described herein.
