RESOLUTION RE: ACTION OF THE BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY, COLORADO, CONCERNING SITE APPLICATION OF GERIATRICS , INC. , D/B/A ARA LIVING CENTERS, AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS , the Board - of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, Geriatrics, Inc. , dba ARA Living Centers, wishes to construct a lift station and force main on the following described parcel of land, to-wit: SEI SE* of Section 9 , Township 5 North, Range 66 West of the 6th P.M. , Weld County, Colorado WHEREAS, the Regulations for Site Applications for Wastewater Treatment Facilities require review of the Site Application by the Board of County Commissioners prior to commencing construction and, further, that various local and State agencies be given the opportunity to review and comment on said Site Application, and WHEREAS , Geriatrics, Inc. , dba ARA Living Centers, has submitted its Site Application to the Board of County Commissioners of Weld County for review and comments, a copy of said Application being attached hereto and incorporated herein by reference, and WHEREAS, after study and review, the Board finds that said Site Application is compatible with the Weld County Comprehensive Plan and that it is in the best interest of Weld County to recommend approval of said Application, subject to the following conditions: 1 . Installation of pump station mains and wastewater facilities shall be in compliance with pertinent sections of the Greeley Municipal Code prior to municipal acceptance of wastewater. 2 . Payment of connection charges and plant investment fees shall be made prior to the new service system connection to the City of Greeley. 3 . The existing septic systems shall be properly abandoned and not used as a back-up system during power outages. 4 . It shall be tested on a regular monthly basis to ensure proper operation and maintenance. 5 . An underground utility permit shall be approved by the Weld County Engineering Department for the installation and construction of the facilities along, across, upon, and under all Weld County Road rights-of-way. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Site Application of Geriatrics, Inc. , dba ARA Living Centers, be, and hereby is, recommended favorably to the Colorado Department of Health as being compatible with the Weld County Comprehensive Plan, subject to the hereinabove mentioned conditions. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said Site Application. Weld County Comprehensive Plan Urban Growth Boundary Goal #2 encourages County and municipalities to coordinate public infrastructure systems within urban growth boundary areas. Public Facility and Service's Policy #6 indicates that facilities and services proposed within a municipality's urban growth boundary should be designed for compatibility with the future municipal infrastructure planned for the area. The City of Greeley Planning Commission, the Weld County Health Department and the Weld County Engineering Department have reviewed this request and recommended approval with conditions. These conditions are listed in the staff's recommendation. This recommendation is based, in part, upon a review of the application submitted by the applicant, other relevant information regarding the request, and the responses of the referral entities which have reviewed this request. The Planning Commission's recommendation for approval is conditional upon the following: 1. The Center as it presently exists has outgrown the existing on-site wastewater treatment facility; namely septic tanks and a leach field. (See Exhibit No. 1, "Site Plan" ) The five existing septic tanks are required to be pumped at least twice a week; therefore, the facility is not efficient or economical in dis- posing of the wastewater. Several wastewater treatment alternatives have been reviewed: 1 ) The parcel of land owned by ARA Dev Con, Inc. , does not contain enough available land to provide sufficient area for leach fields. It is not the intent or desire of the Center to obtain additional land for the leach fields. 2 ) A gravity sewerline to the City of Greeley' s wastewater collection lines is not possible at this time. The elevation of Greeley' s sewerline would not allow positive drainage and proper cover to facilitate a gravity system. The City of Greeley has no plans of providing a collection system in the foreseeable future for this area since it is in unincorporated Weld County. 880270 3 ) The most cost effective and permanent solution consists of a lift station to pump the wastewater through a force main to the City of Greeley' s collection system approximately 3 ,000 feet away. The City of Greeley has agreed to accept the wastewater from the Center. (See Exhibit No. 3, letter from City of Greeley) Location and Population The Center is located in the southeast quarter of the South- east Quarter (SE 1/4, SE 1/4) of Section 9, Township 5 North, Range 65 West of the 6th Principal Meridian, Weld County, Colorado. The Center is physically located at the intersection of East 18th Street (Highway 34 Business Route) and Birch Avenue with a street address of 1819 Birch Avenue. (See Exhibit No. 2, "Vicinity Map" ) The population of fifty-six residence and thirty-five staff members is the maximum number ever anticipated at the Center at one time. The population is not anticipated to increase in the future as the Center does not have additional land for expansion. Design Criteria The lift station and force main is designed for a daily average hydraulic capacity of 9 ,000 gallons per day or 6.25 gallons per minute and an average organic capacity of 16 pounds BOD per day. The maximum hydraulic flow rate, although not measured, is estimated to be about 3.5 times the aveage for short periods of time. Thus, an instantaneous flow rate of up to 20 gpm can be anticipated. The hydraulic loading is based on water useage for the Center as reported by the City of Greeley for the months of April, May, and June, 1987 . The water useage figures include a laundry room, which discharges into a seperate septic tank and leach field that is operating properly. This site application for the lift station includes the laundry wash water to be connected at a later date. Lift Station: The lift station will consist of a 48 inch diameter concrete wet well containing two submersible grinder pumps and the necessary electrical controls to operate the lift station. The pumps will be one horsepower grinder pumps capable of shredding materials generally found in domestic wastewater. Each pump will be capable of delivering 40 gallons per minute against a total head of 27 feet. Existing Sanitary Sewer Service The only sanitary sewer service entity within a five mile radius of the Center is the City of Greeley Wastewater Treatment Plant (See Exhibit No. 2, "Vicinity Map" ) 880270 _ The The Center will be discharging their sanitary sewage into the City of Greeley' s collection system with the assistance of the proposed lift station and force main. As a result, treat- ment of the waste will occur in the Greeley Wastewater Treatment Plant. The pumps will be connected to the Center' s electric source. In case of an electrical power outage, the Center is equipped with a backup generator. The lift station pumps will also be powered by the generator during electrical outages of the primary source. A second backup system consists of 4 inch overflow pipe extended from the wet well to an existing septic tank and leach field. The overflow pipe will prevent the wet well from overtopping should both power sources fail. The wet well will also be equipped with a high water warning system to — alert the operator prior to the use of the overflow pipe. The lift station will not be located in a flood hazard zone as determined by the Flood Insurance Rate Map (FIRM) . Force Main: Three inch PVC Pipe will be used as the force main. The slope of the pipe will be determined by field survey 880270 and shown on the plans prior to the review of the plans and specifications by the Colorado Department of Health. The force main will be buried below frost line and designed to meet all requirements set by the Colorado Department of Health. Operation and Maintenance Plan The pumps will be controlled from float switches in the wet well and operated with an off-on pump sequence; therefore allowing both pumps to be used equal and creating less chance of overheating. All mechanical and electrical devices will be serviced in accordance to the manufacture ' s recommendations by on-site staff members. Financial Analysis The construction, operation, and maintenance of the lift station and force main will be paid in full on a cash basis by Geriatrics, Inc. The construction of the lift station and force main is considered a necessity to insure the operation and main- tenance of the New Life Center. Implementation Plan New Life Center is presently pumping the existing septic tanks twice a week incurring large operational costs. Therefore, Geriatrics, Inc. , will install the lift station and force main as soon as all necessary approvals are received from the Colorado Department of Health. Legal Aspects New Life Center and the parcel of land on which the Center resides is owned and controlled by Geriatrics, Inc. (See Exhibit No. 4, Warranty Deed filed with Weld County Clerk and Recorder October 5, 1984 ) The force main will be crossing three private land owners property. Utility easements from the land owners allowing the force main to be placed across their land are being formally obtained based on the agreements to date. One thousand two hundred feet (1, 200 ' ) of the force main will be placed in Weld County' s right-of-way. (See Exhibit No. 2, "Vicinity Map" ) Institutional Arrangements Geriatrics, Inc. , is the only user; therefore, no association need be involved.
