RE: APPROVE ADDENDUM TO AGREEMENT BETWEEN WELD COUNTY AND
PREFERRED CORRECTIONAL MEDICAL TEAM, INC. , AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Addendum to the
Agreement for Weld County Sheriff' s Office Jail Medical Unit
Service between Weld County and Preferred Correctional Medical
Team, Inc. , dated September 11 , 1988 , said Addendum 'concerning
drug screening twice monthly for all inmates in the work release
program, and
WHEREAS, after review, the Board deems it advisable to
approve said Addendum, a copy being attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that the Addendum to the
Agreement for Weld County Sheriff' s Office Jail Medical Unit
Service between Weld County and Preferred Correctional Medical
Team, Inc. , be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Addendum.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 26th day of
October, A.D. , 1988 .

ADDENDUM
This Addendum is hereby attached to and made a part of that
certain Agreement For Weld County Sheriff 's Office Jail Medical
Unit Service dated September 11, 1988, by and between the Board
of County Commissioners of the County of Weld, 915 Tenth Street,
Greeley, Colorado 80631, (hereinafter "County" ) , and Preferred
Correctional Medical Team, Inc . , 2000 Sixteenth Street, Suite 7,
Greeley, Colorado 80631, (hereinafter "Provider" ) .
The County desires that drug screens be performed twice
monthly for all inmates in the work release program. Additional
tests may be ordered by the County, at its discretion, if it
deems such additional tests advisable .
Provider desires to provide County with drug screen services
and hereby agrees to the following:
I . Provider will furnish the sample kit containers,
deliver to the Jail and pick up specimens .
II . Provider, or its consultant, will provide inservice
training to County personnel obtaining the specimen
from the inmates as requested by Jail personnel .
III . Test results will be delivered to County within two ( 2 )
working days after specimen is picked up by Provider .
IV. Drugs to be tested for are : Amphetamines, Cocaine
Metabolites, Barbiturates, Opiates, Benzodiazepines,
Methaqualone, Propohyphene, Phencyclidines, Methadone,
THC and Alcohol .
V. Provider shall use only laboratories licensed by the
Colorado State Health Department .
VI . Cost of a routine test is $15. 00 per test. If a
specimen tests positive, an extensive confirmation test
is required . The cost of the confirmation test is an
additional $35. 00 .
County, in consideration of Provider ' s promises, agrees to
the following:
I . County personnel will witness the taking of the
specimen from the inmates, seal the specimen bottle,
label and maintain control of the specimen until picked
up by Provider or Provider 's courier services .

II . County agrees to pay provider $15.00 per screen plus an
additional $35. 00 per any confirmation tests required .
Either party may cancel this Addendum by five ( 5) days
written notice to the other party.
The parties hereto have caused this Addendum to be executed
as of this 26th day of October, 1988 .
