Purchase of Services Agreement Between Migrant Head Start and
Subject:
Colorado Department of Health' s Migrant Health Program
Enclosed for Board approval is a Purchase of Services Agreement between
the Weld County Division of Human Resources ' Migrant Head Start Program
and the Colorado Department of Health's Migrant Health Program.
The Colorado Migrant Health Program will provide the following services:
1. Physical examinations for 400 children in the Greeley, Frederick,
Longmont, Fort Collins, and Alamosa Centers will be provided.
Migrant Head Start will reimburse $10.00 per child for the
examination. (400 children x $10.00 per child = $4,000.00)
2. Child Health Associate Intern Services will provide interns to
serve the Greeley Head Start location for the months of July
and August.
Migrant Head Start agrees to reimburse the Health Program a
maximum of $250.00 per month for a maximum of two (2) months
service. ($250.00 per month x 2 months = $500.00)
3. Nursing Services to serve the Greeley, Frederick and Longmont
Head Start Centers will be provided. The total reimbursement
will be a maximum of $11,808.OO.
4. Dental Services will be provided for Longmont, Fort Collins ,
and Alamosa Head Start Centers. The total reimbursement will
be a maximum of $1 ,875.00
5. Medical Care for acute conditions and follow-up on abnormal
screening results will be provided for the Fort Collins and
Alamosa Migrant Head Start children. The total reimbursement
will be a maximum of $1,500.00.
6. Maximum reimbursement will be for a total of $19,6E3.
7. The terms of this agreement is from May 15, 1989 through
October 6, 1989.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this 12th day of July
1989, by and between the Colorado Department of Health's Migrantnt Hearth
Program, hereinafter referred to as "Health Program," and the Weld County
Division of Human Resources ' Migrant Head Start Program, hereinafter referrec
to as "Migrant Head Start."
WITNESSETH
NOW, THEREFORE, in consideration of the mutual covenants herein, the
parties hereto agree as follows:
1. Physical Examinations:
The Health Program will provide physical examinations for 400 Migrant Head
Start children from the Greeley, Frederick, Longmont, Fort Collins, and
Alamosa centers. Migrant Head Start agrees to reimburse the Health
Program a maximum of $10.00 per child for the examination services
($10.00/child x 400 children = $4,000.00) .
2. Child Health Associate Intern Services:
The Health Program will provide Child Health Associate Interns tc serve
the Greeley Head Start location for the months of July and August .
Migrant Head Start agrees to reimburse the Health Program a maximum
of $250.00 per month for Child Health Associate Intern services for a
maximum of two (2) months of service ($250.00/month x 2 months = $500.00) .
3. Nursing Services:
The Health Program will provide a nurse or Child Health Associate Student
to serve the Greeley, Frederick and Longmont Head Start centers during the
time they are in session, June 12, 1989 - October 2, 1989.
Migrant Head Start agrees to reimburse the Health Program a maximum of
$7.38 per hour for these services for a total not to exceed $11 ,808.00.
4. Dental Services:
The Health Program will provide dental services for Longmont, Fort
Collins, and Alamosa Migrant Head Start centers. Migrant Head
Start will reimburse the Health Program a maximum of: $15.00 per
child x 25 children for the Longmont location, $25.00 per child. x.
25 children for the Fort Collins location, and $25.00 per child x
35 children for the Alamosa location ($15.00/child x 25 children =
$375.00, $25.00/child x 25 children = $625.00, and $25.00/child x 35
children = $875.00) , for a maximum total of $1,875.00.
5. Medical Care:
The Health Program will arrange medical care for acute conditions
and follow up on abnormal screening results for the Fort Collins

and Alamosa. Migrant Head Start children. Migrant Head Start agrees to
reimburse the Health Program a maximum of $1,500.00 for this service.
6. Travel :
The Health Program understands and agrees that it is to pay all
travel expenses associated with fulfilling its service obligations
under this Agreement.
7. Maximum Reimbursement:
The total maximum amount to be reimbursed to the Health Program.
under this Agreement is $19,683.00.
8. Term of Agreement:
The term of this Agreement is from May 15, 1989 through October 6,
1989.
9. Amendments :
This Agreement may be amended at any time with the written mutual
consent of the parties hereto.
10. Cancellation:
The Health Program and Migrant Head Start mutually agree to accept
that this Agreement may be cancelled by either party after a thirty
(30) day written notice has been provided to the other party. In
the event funds are withdrawn from the Head Start Program by the
Department of Health and Human Services, this Agreement may be
cancelled immediately.
11. Assurances:
a. The Health Program agrees that it is an independent contractor
and its officers, agents, or employees do not become employees
of Weld County, and therefore are not entitled to any employee
benefits as Weld County employees, as the result of the
execution of this Agreement.
b. The Health Program shall provide adequate liability and workers'
compensation insurance for all its employees, volunteers , and
agents engaged in the performance of this Agreement. Likewise,
Migrant Head Start, by and through the Board of Weld County
Commissioners , shall provide adequate liability and workers '
compensation insurance for all its employees, volunteers, and
agents engaged in the performance of this Agreement.
c. No officer, member, or employee of Weld County, and no member
of their governing bodies, shall have any pecuniary interest,
direct or indirect, in the approved Agreement or the proceeds
thereof.

d. The Health Program understands that monitoring and evaluation
of the performance of this Agreement shall he conducted by the
Weld County Department of Human Resources and the results
provided to the Board of Weld County Commissioners.
e. The Health Program assures that it will comply with Title VI of
the Civil Rights Act of 1986 and that no person shall , on the
grounds of race, creed, color, sex, or national origin, be
excluded from participation in, be denied benefits of, or be
otherwise subjected to discrimination under the approved
Agreement.
IN WITNESS WHEREOF, the parties hereunto have caused this Agreement
to be duly executed as of the day, month, and year first hereinabove set
forth.
