Subject: Purchase of Service Agreement w/ School District RE- J
Enclosed for Board approval is the Purchase of Service Agreement between
the Weld County Head Start Program and Weld County School District
RE-5J.
The Head Start Program will utilize the classroom space at the School
District's Letford Trailer Site in Johnstown, Colorado. The classroom
space will cost the regular Head Start Program $160.00 per month. This
is a $10.00 increase in rent from last year.
The School District will provide transportation for all Head Start
children who attend the Johnstown Center. Those children who live on
the regular School District bus routes will be transported for free.
Head Start will pay the School District $8.00 per month per child for
transporting all children who do not live on the School District's
regular bus routes.
The School District also agrees to provide hot lunches to the Head Start
children and adults for $1.35 per meal . This is an increase of $ .10
per meal .
This agreement is effective from September 19, 1985, through May 31,
1986.

PURCHASE OF SERVICE AGREEMENT
THIS AGREEMENT, made and entered into this 14th day of August,
1985, by and between the Weld County Division of Human Resources' Head
Start Program, hereinafter referred to as "Human Resources" , and the
Weld County School District RE-5J, hereinafter referred to as "School
District".
WITNESSETH:
Thai: for and in consideration of the covenants and agreements
herein agreed to be kept and performed, the School District hereby
leases and agrees to provide unto Human Resources the following
described premises and services upon the following terms and conditions:
1. The School District agrees to provide classroom space at the
Letford Trailer Site, Johnstown, Colorado, for the rental
amount of $160.00 per month. The classroom space will be made
available for one (1) daily Head Start session, beginning
September 19, 1985, and ending May 31, 1986, contingent upon
the School District's state requirements.
Billing for rent shall be made monthly and may be made with the
school lunch billing referenced in item 3 below.
2. The School District agrees to provide bus transportation for
D- - - - - the Head Start children who live on the regular bus routes
within the School District's boundaries. There will be no
transportation charge for these children.
The Head Start children who do not live on the regular bus
route will be provided transportation by the School District.
There will be a transportation charge of $8.00 per child per
month, not to exceed the total charge of $450.00 during the
time frame referenced in item 1 above.
Billing for transportation shall be made monthly and may be
made with the school lunch billing referenced in item 3 below.
3. The School District agrees to provide hot lunches to the Head
Start classroom at the price of $1.35 per meal . The School
District will submit a bill to Human Resources by the 10th of
the month for all services (space/transportation/meals)
provided the preceding month.
4. The School District agrees to provide meals in accordance to
the regulations of the United States Department of Agriculture's
Child Care Food Program as shown in Attachment A of this Agreement.
5. Human Resources will notify the School District within the
first hour of the beginning of the school day if there is a
variance in the number of meals needed for that day. Human
Resources will notify the School District the day before
holidays and vacations that meals will not be needed for those
specified day(s).

6. Thorough and complete custodial care shall be provided by the
School District daily. This service is .provided as part of the
rental agreement stipulated in item 1.
7. Human Resources agrees to provide telephone service for the
Human Resources ' Head Start staff for any telephone installed
in the Letford trailor.
8. Human Resources agrees to pay the above mentioned and agreed
upon price for all classroom space, transportation and meals
served to Head Start children, staff and volunteers. The
reimbursement for all such services will be made within three
(3) weeks of receipt of a statement of charges from the School
District.
9. Human Resources agrees that it will throughout the term of this
Agreement comply with the rules and regulations of the School
District in effect from time to time as they may affect the
Head Start children.
10. It is mutually agreed that this Agreement may be modified,
provided a thirty day written notice is provided to the other
party. Any modifications made to this Agreement must be agreed
upon in writing and signed by appropriate officials.
11. It is mutually agreed that this Agreement may be cancelled by
either party providing the other party with a minimum of 30
days written notice. This Agreement will terminate immediately
in the event of a loss of funding to the Human Resources' Head
Start Program.
IN WITNESS WHEREOF, the parties hereunto have caused this Agreement
to be duly executed as of the day and year first hereinabove set
forth.
