Subject: Amendments to Purchase of Service Agreements with RE-1J & RE-5J
Enclosed for Board approval are amendments to the Purchase of Service
Agreements between Head Start and School Districts RE-1J and RE-5J.
As you recall , the above mentioned school districts provide the hot
lunches and snacks for the Head Start Centers located in Firestone and
Johnstown.
The U.S.D.A. has requested additional wording be added to each
agreement. The amended agreement will now guarantee that each school
district will maintain the menu/production records , meal counts, and
food/milk invoices for audit and review purposes by Head Start.
Each of the school districts are currently maintaining the records for
their own U.S.D.A. reimbursement purposes. The amendment will not
increase their work load or the meal prices charged to Head Start.
The original agreements which were approved in September, 1985, remain
in effect.

AMENDMENT TO
PURCHASE OF SERVICES AGREEMENT
THIS AMENDMENT, made and entered into this 16th day of October,
1985, by and between the Boulder County School District RE-1J, located
at the Frederick Junior/Senior High School , hereinafter referred to as
"School District" , and the Weld County Division of Human Resources' Head
Start Program, hereinafter referred to as "Head Start".
WITNESSETH
WHEREAS THE SCHOOL DISTRICT AND HEAD START are desirous of amending
the original agreement of September 16, 1985.
NOW THEREFORE, the following conditions are mutually agreed upon:
1. The School District will maintain the following records for
Head Start:
a. Menu/Production Records, including number of meals
prepared, menu, portion sizes of menu items and
amounts of food used to prepare menu items.
b. Meals including daily number of meals delivered by
type.
c. Food and milk invoices.
The records will be made readily available to Head Start for
audit and review purposes. The School District will maintain
the records within the record retention guidelines mandated by
U.S.D.A.
2. The School District will report the items listed in Section 1
above to Head Start by the 10th of every month.
3. All terms and conditions of the original agreement shall remain
in effect unless otherwise stated in this agreement.
IN WITNESS WHEREOF, the parties hereunto have caused this amendment
to be duly executed as of the day and year first hereinabove set forth.

AMENDMENT TO

"School District" , and the Weld County Division of Human Resources ' Head
Start Program, hereinafter referred to as "Head Start".
