Dear Mr. Lacy:
Based on our review of the bid opening documentation which you submitted
for the above referenced project, your proposed award of a contract to
the following low bidder is hereby approved:
CONTRACTOR TYPE OF WORK BID
Hall-Irwin Construction Water, Sewer and Streets $1,245,906.65
Weld County Base for Streets $ 105,953.68 (Est. )
TOTAL $1,351 ,860.33
Please note that EDA will participate in the cost of the street base materials
furnished by the County with proper documentation and approval of a reasonable
unit cost for the materials.
To obtain EDA approval of the executed contract documents, please submit to
this office one copy of the following for each contract.
1. All documents furnished the bidder prior to the receipt of
bids, including all addenda issues upon which the bids were
based, if not previously submitted.
2. A copy of the executed contract or agreement between the
grantee and the contractor, having all necessary blanks
completed.
3. A copy of the performance and payment bonds of the contractor(s) ,
dated the same or subsequent to the date of the contract,
supported by a properly signed power of attorney issued by
the Surety.
4. A copy of the Certificate of Insurance which the contractor
must carry in conformance with the contract requirements.
5. Executed copy of Form ED-126, Attorneys Certification of
Executed Contract, or acceptable equivalent Statement of
completeness and legality of the contract.

In response to the requirements of E.O. 11246, you are required to give
written notice to the Director, Office of Federal Contract Compliance
Programs (OFCCP); of the award of this/these contract(s). The notifi-
cation should be sent to the following address:

for OFCCP/ESA
U. S. Department of Labor

Denver, Colorado 80204
It should identify the EDA Project as referenced above, and include the
name, address and telephone number of the/each contractor(s); employer
identification number(s); dollar amount of the contract(s); estimated
starting and completion date(s) of the contract(s); the contract number(s)
and the geographical area in which the contract(s) is/are to be performed.
A copy of this notification should be sent to this office for our files.
Once construction has started the contractor(s) is/are required to submit
weekly payroll reports. The payroll reports may be submitted on the
standard form WH-347 or on the contractors own form provided it contains
all the information required on form WH-347 . If the contractors own form
is used, a Certificate of Compliance, form ED-162 will need to be submitted
with each payroll. EDA will supply samples of either of these forms upon
request.
