Subject: 5 Purchase of Service Agreements for Migrant Head Start
Enclosed for Board approval are 5 Purchase of Service Agreements for the
Migrant Head Start.
1. Sunrise Community Health Center: The Health Center will provide
medical services twice a week at the Greeley Head Start Center.
The agreement runs from July 15, - October 31. The
medical services will cost $2,000.00.
2. Sunrise Community Health Center: The Health Center will provide
services to Migrant Head Start at the rate of $25.00 per
child. The agreement runs from July 1, - October 31.
3. Plan de Salud del Valle: The Health Center will provide medical
services to the Lafayette Migrant Head Start Center from July 15, -
October 31. Head Start will pay the Health Center $20.00 per child
not to exceed $2,000.00 for these services.
4. Plan de Salud del Valle: The Health Center will provide dental
services for Migrant Head Start children at the rate of $25.00 per
child. The agreement runs from July 1, - October 31.
5. Colorado Department of Health/Migrant Health Program: The state
program will conduct the physical and dental screenings on all Weld
County Migrant Head Start children. By entering into this agreement
with the state program, the Head Start Program is able to contain
the costs for medical and dental services outlined in the above
agreements. The cost for all screenings and nutrition services is
$?,640.00. The agreement runs from July 18, - October 31.

Purchase of Services
THIS AGREEMENT, made and entered into this 14th day of August,
1985, by and between the Sunrise Community Health Center, herinafter
referred to as "Health Center" , and the Weld County Division of Human
Resources' Migrant Head Start Program, hereinafter referred to as "Head
Start".
WITNESSETH
That for and in consideration of the covenants and agreements
herein agreed to be kept and performed, the Health Center and Head Start
agree to provide the following described services based upon the terms
and conditions outlined below:
1. The Health Center shall provide the health services for the
Head Start center located at 520 13th Avenue, Greeley,
Colorado, 80631.
2: The Health Center shall provide the services of a Health
Associate to Head Start. The Health Associate will be present
at the Head Start center twice a week. The Health Center
will be responsible for the Health Associate's mileage to and
from the Head Start center.
3. The Health Center agrees to provide all medical examinations,
medical treatment, lab work, x-rays, etc. , as is deemed
necessary for any referred Head Start child.
4. Head Start agrees to reimburse the Health Center $2,000.00 for
services provided during the term of this contract. The above
cited rate will pay for all medical examinations, lab work,
x-rays , etc. Prescriptions will not be covered in the above
cited rate.
5. The Health Center will not perform routine physical or dental
examinations on the Head Start children.
6. The term of this agreement is from July 15, 1985 through
October 31 , 1985.
7. This agreement may be amended at anytime with the written
mutual consent of both parties.
8. The Health Center and Head Start mutually agree that this
agreement may be cancelled by either party after a thirty day
written notice has been provided the other party.

9. This agreement will be terminated immediately in the event
funding for the Migrant Head Start program is stopped.
IN WITNESS WHEREOF, the parties have caused this agreement to be
duly executed as of the day and year herinabove set forth.

PURCHASE OF SERVICE
THIS AGREEMENT, made and entered into this 19th day of August,
1985, by and between the Weld County Division of Human Resources'
Migrant Head Start Program, hereinafter referred to as "Head Start" and
the Sunrise Community Health Center, hereinafter referred to as "Health
Center" .
WITNESSETH
In consideration of the mutual promises and covenants contained
herein, Head Start and the Health Center agree as follows:
1. Head Start agrees to transport the Head Start children to the
Health Center to meet the scheduled dentist appointments.
2. The Health Center shall provide prophylaxis therapy and
application of topical flouride, for each Head Start child.
The Health Center agrees to provide all restoration and/or
extractions as is deemed necessary on each referred child.
3. Head Start agrees to reimburse the Health Center $25.00 per
child for these services.
4. Head Start will not be charged for those children covered by
Public Assistance. Medicaid numbers shall be provided at the
time of the appointment and billing shall be sent to the
appropriate agency by the Health Center.
5. The term of this agreement is from July 1, 1985, through
October 31, 1985.
6. Both parties agree that this agreement may be modified upon the
written consent of both parties.
7. Both parties agree that this agreement may be terminated upon
thirty days written notice from one party to the other,
however, this agreement will be terminated immediately if
funding for the Weld County Migrant Head Start Program is
terminated.
IN WITNESS WHEREOF, the parties above hereunto placed their hands
as of the date written above.

Migrant Head Start Program, hereinafter referred to as "Head Start".

herein agreed to be kept and performed, the Health Center and Head Start
agree to provide the following described services based upon the terms
and conditions outlined below:
1. The Health Center shall provide the health services for th.e
Head Start center located at 805 Excalibur Street, Lafayette,
Colorado, 80026.
2. The Health Center shall provide the services of a Health
Associate to Head Start. When available, the Health
Associate will be present at the Head Start center on a daily
basis. The Health Center will be responsible for the Health
Associate's mileage to and from the Head Start center. \\

3. The Health Center agrees to provide all medical examinations,
medical treatment, lab work, x-rays , etc. , as is deemed
necessary for any referred Head Start child.
4. Head Start agrees to reimburse the Health Center at the rate
of 520.00 per child not to exceed a total reimbursement of
52,000.00. The above cited rate will pay for all medical
examinations, lab work, x-rays , etc. Prescriptions will not
be covered in the above cited rate.
5. The Health Center will not perform routine physical or dental
examinations on the Head Start -children.
6. The term of this agreement is from July 15, 1985 through
October 31 , 1985.
7. This agreement may be amended at anytime with the written

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this day of July, 1985,
by and between the Colorado Department of Health igrant Health
Program, hereinafter referred to as "Health Program" , and the Weld
County Division of Human Resources ' Migrant Head Start Program,
hereinafter referred to as "Head Start".
WITNESSETH
That for and in consideration of the covenants and agreements
herein agreed to be kept and performed, the Health Program agrees to
provide Head Start the following described services upon the following
terms and conditions:
1. The Health Program will provide a physical examination for 200
migrant Head Start children. The physical examination will
include the following:
A. Physical Examination
B. Hemocrit
C. Hearing Screening
D. Dental Screening
2. Head Start agrees to reimburse the Health Program a maximum
cost of 510.00 per child for the physical examination services.
3. The Health Program will provide nutrition services to Head
Start. A nutritionist will be on call . The nutritionist will k.
work with the cooking staff in preparing special diets for tt
children with special dietary needs , providing nutrition
education in the classrooms and giving presentations to the
families during "family nights" .
4. Head Start agrees to reimburse the Health Program a total of
5600.00 for the nutrition services .
5. The Health Program will provide a dental hygienist to conduct
dental screenings. The dental hygienist will screen each Head
Start child, prioritize the child' s need for treatment,
determine how many children will need to have appointments with
the dentist, determine the order in which the children will be
referred to the dentist, and estimate the number of dental
appointments that will be needed per child.
6. Head Start agrees to reimburse the Health Program a total of
51 ,040.00 for the dental hygienist and the services provided.

7. The Health Program understands and agrees to provide all the
services cited in this agreement to the participants of the
Head Start centers located in Greeley, Colorado and Lafayette,
Colorado. The Health Program will pay all associated travel
expenses.
8. The total amount to be reimbursed to the Health Program is
$3,640.00.
9. The Health Program should receive remuneration for the services
on or before October 31 , 1985. The check should be made
payable to the Colorado Department of Health/Migrant Health
Program.
10. The term of this agreement is from July 18, 1985, through
October 31, 1985.
11. This agreement may be amended at anytime with the written
mutual consent of both parties.
12. The Health Program and Head Start mutually agree to accept that
this agreement may be cancelled by either party after a thirty
(30) day written notice has been provided the other party. In
the event funds are withdrawn from the Head Start Program by
the Department of Health and Human Services , this agreement may
he cancelled immediately.
IN WITNESS WHEREOF, the parties hereunto have caused this agreement
to be duly executed as of the day and year first hereinabove set forth.
