Amendments are in response to an additional allocation of Older American's Act funds authorized by the Board on October 6, 1988. . Purchase of Services Agreement Amendment between the Area Agency on Aging and Eldergarden Adult Day Program to increase the FY 1988 award from $10,000 to $11,000 to provide additional resources to open a satellite adult day program in Fort Lupton to better serve south Weld County. . Purchase of Services Agreement Amendment between the Area Agency on Aging and Weld Mental Health Center to increase the FY 1988 award from $23,204 to $25,524 to increase peer counseling units from 4,500 to 5,000 units of service. . Purchase of Services Agreement Amendment between the Area Agency on Aging and Meals on Wheels to increase the FY 1988 award from $33,400 to $39,690 to increase the number of home delivered meals from 23,332 to 26,832. . Purchase of Services Agreement Amendment between the Area Agency on Aging and Catholic Community Services Northern to increase the FY 1988 award from $23,258 to $25,584 to develop a volunteer support program in order to reach more Hispanic Elders in rural areas of Weld County. . Purchase of Services Agreement Amendment between the Area Agency on Aging and Catholic Community Services Northern to increase the FY 1988 award to Senior Companions from $7,753 to $8,528. If you have any questions , please telephone Linda Piper at 353-0540. 11 #17 e oo—i) r PURCHASE OF SERVICES AGREEMENT AMENDMENT THIS AGREEMENT, made and entered into this 12th day of October, 1988, by and between Weld County Area Agency on Aging, Division of Human Resources, Weld County Government, hereinafter referred to as "AAA," and Catholic Community Services Northern, hereinafter referred to as "CCSN." WITNESSETH WHEREAS, AAA has been issued a revised grant award from the State Department of Social Services for FY 1988, and WHEREAS, AAA and CCSN are desirous of amending the original agreement of December 30, 1987, to award additional monies to CCSN for outreach services to Spanish surname elderly. NOW THEREFORE, it is mutually agreed: 1. The AAA will increase the Older Americans Act monies awarded to CCSN from $23,258 to $25,584. 2. CCSN will utilize the additional funds to develop a volunteer support program in order to reach more seniors in the rural areas of Weld County. 3. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. Piper, Director 881167 PURCHASE OF SERVICES AGREEMENT AMENDMENT THIS AGREEMENT, made and entered into this 12th day of October, 1988, by and between the Weld County Area Agency on Aging, Division of Human Resources, Weld County Government, hereinafter referred to as "AAA," and Meals on Wheels of Greeley, hereinafter referred to as "MOW." WITNESSETH WHEREAS, AAA has been issued a revised grant award from the State Department of Social Services for FY 1988, and WHEREAS, AAA and MOW are desirous of amending the original agreement of December 30, 1987, to award additional monies to MOW. NOW THEREFORE, it is mutually agreed: 1. The AAA will increase the Older Americans Act monies awarded to MOW from $33,400 to $39,690. 2. MOW will increase the number of meals delivered to the homebound elderly from 23,332 to 26,832. 3. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. WITNESSETH WHEREAS, AAA has been issued a revised grant award from the State Department of Social Services for FY 1988, and WHEREAS, AAA snd Weld Mental Health Center are desirous of amending the original agreement of December 30, 1987, to award additional monies to Weld Mental Health Center. NOW THEREORE, it is mutually agreed: 1. The AAA will increase the Older Americans Act monies awarded to Weld Mental Health Center from $23,204 to $25,524. 2. Weld Mental Health Center will increase outreach counseling from 4500 units to 5000 units to 160 individuals, and increase mental health consultation and education from 75 units to 85 units for 750 individuals. 3. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. NOW THEREFORE, is is mutually agreed: 1. The AAA will increase the Older Americans Act monies awarded to Eldergarden from $10,000 to $11,000. 2. Eldergarden will provide additional personnel resources to open and maintain satellite adult day care services for residents of south Weld County through the Fort Lupton satellite adult day center. 3. All other terms and conditions of the original agreement shall remain in effect unless othewise stated in this Amendment.
