Subject: Purchase of Services Agreement between Weld County Human Resources
and Greeley Transitional House
Enclosed for Board approval is a Purchase of Services Agreement
Amendment between Weld County Division of Human Resources and Greeley
Transitional House.
This amendment extends the term of the original Agreement dated February
29, 1988, to April 30, 1989, and awards an additional amount of $3,000.
These monies will be used to cover the operational costs of providing
essential services to homeless individuals.

Purchase of Services Agreement Amendment
THIS AGREEMENT, made and entered into this 11th day of January, 1989,
by and between the Board of County Commissioners of Weld County, Colorado, by
and through the Weld County Division of Human Resources, hereinafter referred
to as "HRD," and Greeley Transitional House, hereinafter referred to as
"Transitional House."
WITNESSETH
WHEREAS, HRD has applied for and been awarded Emergency Shelter Grant
funds through the Stewart B. McKinney Homeless Assistance Act of 1987, and
WHEREAS, HRD desires to provide emergency shelter renovation funds to
eligible entities who are providing essential services to homeless
individuals, and
WHEREAS, HRD and Transitional House are desirous of amending the original
agreement of February 29, 1988, to award additional monies to Transitional
House and extend the term of the aforementioned agreement.
NOW, THEREFORE, it is mutually agreed:
1. HRD will award an additional $3,000.00 to the initial award of $13,112.00
to cover operational costs of providing essential services to homeless
individuals .
2. The term of the agreement will be extended to April 30, 1989.
3. Transitional House agrees to furnish in-kind match for the additional
$3,000.00 in accordance with the terms of the original agreement. This
in-kind match and its appropriate documentation must be provided to and
received by HRD no later than April 30, 1989.
4. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment
on the day first written above.
