Subject. Modification to Agreement with Home Health Care Services , Inc.
Enclosed for Board approval is a modification to the Purchase of Service
Agreement between the Weld County Area Agency on Aging and Home Health
Care Services.
Home Health Care Services has changed its name to Home Care Services
Inc. The modification is necessary in order to reflect the name change.
Services provided by the organization to the Weld County Area Agency on
Aging will not change.

PURCHASE OF SERVICES AGREEMENT
THIS AMENDMENT, made and entered into this 28th day of April , 1986,
by and between the Weld County Division of Human Resources' Area Agency
on Aging, hereinafter referred to as "AAA" and Home Health Care Services
Inc. , hereinafter referred to as "HHCS".
WITNESSETH
WHEREAS, AAA and HHCS are desirous of amending the original
agreement of September 30, 1985.
NOW THEREFORE, the following conditions are mutually agreed upon:
1. The AAA agrees to recognize and fund Homemaker services under
the new non-profit coporation title of Home Care Services Inc. ,
formally recognized and funded as Home Health Care Services
Inc.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
