RE: APPROVE AMENDMENT NO. VIII TO THE HEALTH PLAN DOCUMENT OF THE
WELD COUNTY EMPLOYEE BENEFIT FUND AND AUTHORIZE CHAIRMAN TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment No. VIII
to the Health Plan Document of the Weld County Employee Benefit
Fund, and
WHEREAS, after study and review, the Board deems it advisable
to approve Amendment No. VIII to said Plan , a copy of which is
attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. VIII to
the Health Plan Document of the Weld County Employee Benefit Fund
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is , authorized to sign said Amendment.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 26th day of
November, A.D. , 1986.

AMENDMENT NO. VIII
to the Health Plan Document of
THE WELD COUNTY EMPLOYEE BENEFIT FUND
The Health Plan Document of Weld County Employee Benefit Fund is hereby
amended, effective January 1, 1987, as follows:
The following is hereby added in its entirety:
Under Covered Expenses:
13. charges for birth control pills and devices
14. charges for mammograms
Under Summary of Benefits:
Routine Physical Benefit $100, including x-rays and lab
work, for Covered Employee
only, per Calendar Year. Not
subject to deductible.
IT IS AGREED BY WELD COUNTY that the provisions contained in the Plan
Document and Amendment No. VIII thereto are acceptable and will be the
basis for the administration of said Employer's Employee Benefit Program
described herein.
