subject: Medical & Dental Agreements Between Head Start & Sunrise Clinic
Enclosed for Board approval are two Purchase of Service Agreements for the
regular Head Start Program.
1. Dental Agreement: The Sunrise Community Health. Center will provide
dental examinations on each Head Start child. In addition, the clinic
will provide the required dental therapy, restoration, extraction and
application of flouride as needed for each child.. Head Start will
reimburse the clinic $25.00 for each child who receives dental services.
The agreement runs from September 1, 1985, through May 31, 1986.
2. Medical Agreement: The Sunrise Community Health Center will provide
physical examinations and all medical treatment for the Head Start
children. Head Start will reimburse the health center a maximum cost
of $20.00 per child for all medical/physical services.. The agreement
runs from September 1, 1985, through May 31, 1986.
In both cases, the agreements are a continuation of similar purchase of
service agreements that were operated during the previous school year.

PURCHASE OF SERVICES
THIS AGREEMENT, made and entered into this 21st day of October,
1985, by and between the Sunrise Community Health Center, hereinafter
referred to as "Health Center" , and the Weld County Division of Human
Resources' Head Start Program, hereinafter referred to as "Head Start".
WITNESSETH
That for and in consideration of the covenants and agreements
herein agreed to be kept and performed, the Health Center and Head Start
agree to provide the following described services based upon the terms
and conditions outlined below:
1. The Health Center shall provide a physical examination on each
referred Head Start child. The physical examination shall
consist of the following:
a. Physical Examination
b. Hemocrit
c. Urine Analysis
2. In addition to providing a physical examination, the Health
Center agrees to provide all medical treatment, lab work,
x-rays, etc. , as is deemed necessary for any referred Head
Start child.
3. The Health Center agrees to provide immunizations and
additional follow-up tests as is deemed necessary on each
referred Head Start child.
4. Head Start agrees to reimburse the Health Center a maximum cost
of $20.00 per child for all medical/physical services (includes
physical examination, medical examinations , lab work, x-rays,
etc. ).
5. Reimbursement for prescriptions and immunizations will be the
responsibility of Head Start.. This reimbursement is in
addition to the cost for medical/physical services.
6. Head Start shall not be charged for any Head Start child who is
covered by Medicaid. Medicaid numbers shall be provided to the
Health Center at the time of the set appointment and billing
shall be sent to the appropriate agency by the Health Center.

7. Head Start agrees to transport Head Start children to the
Health Center to meet scheduled appointments at the Health
Center.
8. The term of this agreement is from September 1, 1985, through
May 31, 1986.
9. This agreement may be amended at anytime with the written
mutual consent of both parties.
10. The Health Center and Head Start mutually agree to accept that
this agreement may be terminated by either party after a thirty
day written notice has been given the other party. However,
this agreement will be terminated immediately if Head Start
funding should be stopped.
IN WITNESS WHEREOF, the parties hereunto have caused this agreement
to be duly executed as of the day and year first hereinabove set forth.

PURCHASE OF SERVICES

Health Center to meet the scheduled dentist appointments.
2. The Health Center shall provide a dental examination on each
referred Head Start child.
3. The Health Center shall provide prophylaxis therapy and
application of topical flouride for each Head Start child. The
Health Center agrees to provide all restoration and/or
extractions as is deemed necessary on each referred child.
4. Head Start agrees to reimburse the Health Center $25.00 per
child for these services (as outlined in items 2 and 3 above).
5. Head Start will not be charged for those children covered by
Public Assistance. Medicaid numbers shall be provided at the
time of the appointment and billing shall be sent to the
appropriate agency by the Health Center.
6. All dental laboratory work must be approved by the Head Start
Director, in writing, before any dental laboratory work may be
performed. All dental laboratory work will be reimbursed
according to the fee scales of the Health Center. Payment for
all dental laboratory work performed will be the responsibility
of the Head Start Parent and/or Medicaid. Head Start is not
responsible for the payment of any dental laboratory services.
7. The term of this agreement is from September 1, 1985, through
May 31, 1986.

8. Both parties agree that this agreement may be modified upon the
written consent of both parties.
9. Both parties agree that this agreement may be terminated upon
thirty days written notice from one party to the other,
however, this agreement will be terminated immediately if
funding for the Weld County Head Start Program is terminated.
IN WITNESS WHEREOF, the parties above hereunto placed their hands
as of the date written above.
