sun;.ct: Speech Therapy for Regular Head Start - Greeley Center
Enclosed for Board approval is an agreement between Weld County Head
Start and the Speech-Language Pathology Clinic of the University of
Northern Colorado.
The University is agreeing to provide speech and language therapy to all
Head Start children in the Greeley center who have a need that might
impair their communication or learning.
Head Start agrees to pay the University $10,000 for the services
provided throughout the 1985-1986 school year.
The University will do the following from July 17, - May 30, 1986:
1. Conduct a DIAL screening for 300 children.
2. Provide diagnostic speech evaluations for 35 children.
3. Conduct 300 Group Therapy Sessions at 45 minutes per session.
4. Provide 275 children with an individual therapy session that is 45
minutes per session.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this 17th day of July 1985, by and
between the Weld County Division of Hu.4nan Resources ' Head Start Program,
hereinafter referred to as "Head Start," and the University of Northern
Colorado by and on behalf of the Speech-Language Pathology Clinic , hereinafter
referred to as "UNC. "
WITNESSETH:
WHEREAS, Head Start is required by law to provide diagnostic and
therapeutic services addressing speech and hearing disorders of Head Start
children; and
WHEREAS, UNC has the staff and facilities to provide such services, and
WHEREAS, the parties wish to establish an on-going policy for the
provision of services; and
WHEREAS, the parties desire to reduce the terms of their agreement to
writing;
NOW THEREFORE, in accordance with the mutual benefits, responsibilities,
and considerations set forth herein, the parties hereto convenant and agree as
follows:
1 . Scope and Purpose
The purpose of this Agreement is to establish the amount, terms, and
conditions of a Purchase of Services Agreement between Head Start and UNC.
2. Services to be Provided
UNC agrees to perform the following services for Head Start:
A. Provide speech and language therapy to children with a specific speech
or language need that might impair communication or learning. This
therapy will be, when possible, in the dominant language of the child.
B. - Observe those children in the classroom before initiating therapy.
C. - Set up therapy schedule in cooperation with the Head Start Program.
D. - Develop for each child in therapy a written, individualized plan
stating goals and objectives for speech-language therapy.
E. - Submit a copy of the individualized plan to the Head Start teacher
and a copy to the Special Needs Coordinator. A copy will also be
kept on file at the University Speech-Language Pathology Clinic.

F. - Consult with teachers of the children receiving therapy concerning
carry-over activities.
G. - Communicate with parents through staff meetings and/or parent-
teacher conferences. Inform the parents of the content of the
individualized plan, the times for therapy sessions, and the
progress of therapy.
H. - Conduct one-to-one therapy for those children with the most severe
needs.
I. - Conduct group therapy sessions for those children not seen
individually.
J. All diagnostic services, screenings and therapy shall be conducted
by students enrolled in the Speech-Language Pathology program under
the supervision of a certified speech-language pathologist.
3. Budget Statement
Head Start will reimburse UNC on the following basis per academic year:
Weld County, Division of Human Resources, will reimburse the UNC
Speech-Language Pathology Clinic a total of $10,000.00 for services
provided during the 1985-86 academic year which fee represents an actual
cost basis for services of $15,000 less a thirty-three (33%) percent
discount. Weld County Division of Human Resources will reimburse UNC for
services only after receipt of documentation from UNC specifying the
number and kind of services delivered. Documentation (billing) will be
presented on the following dates: December 20, 1985; March 30, 1986; and
May 30, 1986, and reimbursement will be made within 30 calendar days of
the billing payable to the UNC Speech-Language Pathology Clinic. Weld
County Division of Human Resources will reimburse the Clinic Supervisor
and UNC clinicians mileage for travel to the Head Start centers at 20¢ per
mile. Total reimbursement shall not exceed $600.00 per academic year
(September-May).
The fee covers all of the actual cost for the following services:
- Dial Screening for 300 children.
- Diagnostic Speech-Language Evaluations for 35 children.
- Speech and Language Therapy: 300 Group Therapy Sessions at 45
minutes per session.
- Individual Therapy Sessions for 275 children at 45 minutes per
session.

- Mileage reimbursement for travel to county Head Start center at .20¢
per mile not to exceed $600.00.
- Consultant Fee of UNC Clinical Sessions (20 hours)
— Exhibit A is the description of consultant work to be performed and
said Exhibit is incorporated herein by this reference.
4. Terms of Agreement
A. The term of this agreementfshall be for one academic year to commence
on July 17, 1985 and terminate on May 30, 1986.
B. This agreement shall be subject to review and renewal by the parties
annually. Either party may terminate this agreement without cause by
giving ninety (90) calendar days written notice to the other party of
the intention to terminate. Termination may also result automatically
upon loss of federal funding to the Head Start program. In case of
prior termination the payment due for services will be pro-rated
according to actual services provided.
C. This contract shall be construed in accordance with the laws of the
State of Colorado.
D. It is mutually agreed that any lawsuits or causes of action concerning
this agreement shall be renewed in the courts of Weld County,
Colorado, to the extent that those courts are reposed with subject
matter jurisdiction. Both parties agree to submit themselves to the
personal jurisdiction of those courts.
E. This document and all documents incorporated herein by reference,
contains the entire agreement between the parties and supersedes all
other prior written or oral agreements or representations.
F. Both parties agree that they shall conduct their activities under the
auspices of this agreement in accordance with state and Federal
non-discrimination laws.
G. Any waiver by either party of any provision of this agreement at any
one time shall not constitute a waiver of any other or all
provisions. Any waiver by any party of provisions of this agreement
at any one time shall not constitute a waiver for all times.

IN WITNESS WHEREOF, the parties hereto have executed this Agreement on the
day first written above.

EXHIBIT A
Consultations by the UNC Clinical Supervisor to assist in the coordination and
implementation of the speech and language contract will be as follows:
A. Provide during the year, two (2) information workshops for parents and
teachers.
B. Attend two (2) of the Parenting classes during the year to provide parents
with speech and language information.
C. Attend quarterly meetings with teaching staff to discuss overall
effectiveness of the speech and language therapy.
D. Attend open house in the fall to answer any questions parents might have
regarding speech and language therapy for their child.
E. Make home visits as requested by Special Needs Coordinator.
F. Pre-assessment team meetings.
