Enclosed for Board approval are three (3) copies of an agreement between
the Weld County Area Agency on Aging and the Retired Senior Volunteer
Program.
This agreement transfers total responsibility for the coordination of
the Volunteer Medigap Insurance Consultant Program to RSVP. This
responsibility has been shared by the Area Agency on Aging and RSVP.
Also involved in the transfer is $2,511.25 which remain of an award from
Mountain Bell to support training and publicity expenditures related to
the program.

PURCHASE OF SERVICE AGREEMENT
THIS AGREEMENT, made and entered into this 25th day of March 1987, by and
between Weld County Area Agency on Aging, Division of Human Resources, Weld
County Government, hereinafter referred to as "AAA" , and Retired Senior
Volunteer Program, hereinafter referred to as "RSVP".
WHEREAS, AAA develops and administers the Area Plan for a comprehensive
and coordinated system of services, and
WHEREAS, AAA desires to provide opportunities for older persons to
volunteer in meaningful activities, and
WHEREAS, AAA desires to provide a service that will assist older persons
with Medigap Insurance Claims , and
WHEREAS, RSVP has assisted the AAA recruit volunteers for the Volunteer
Medigap Insurance program through a mutual effort for several years ,
NOW THEREFORE, it is mutually agreed:
1. The AAA hereby agrees to relinquish all responsibilities related to
training and coordination of the Weld County Medigap Insurance Program to RSVP.
2. The AAA agrees to relinquish to RSVP management and control of
$2,511.25 of Mountain Bell monies awarded to the Weld County Medigap Volunteer
Insurance Consultant Program in 1985 and 1986 respectively.
3. The AAA agrees to continue to make appropriate referrals RSVP and to be
available for technical assistance and consultation.
4. RSVP agrees to coordinate all aspects of the Volunteer Medigap
Insurance Program to include recruitment and training of volunteers,
establishment of sites where volunteers and clients can meet for consultations,
and publicity of the program to the community at large.
5. RSVP agrees to expend the Mountain Bell monies according to the
original purposes of the grant, that is primarily to meet training and publicity
expenses related to the program.
IN WITNESS WHEREOF, the parties hereto have executed this agreement on the
day first written above.
