THE TRI-AREA AMBULANCE DISTRICT ) CRDER AND DECREE
This matter coming on to be heard in open Court , and it
appearing that Notice of ,Hearing on the Petition to organize the
above District was duly published and mailed in compliance with
the requirements of law.
And it further appearing that by Order of this Court and
Notice issued by the Clerk of the District Court , and published
according to law, there was conducted a Special Electior Marcn
5 , 1985 , and as a result organization of this District was
approved and a Board of Directors elected ; and the Court heir.'

ORDER AND DECREE
The Court has jurisdiction in this matter . Said Distract
has been duly organized prsuant to the "Special District Act"
contained at Title 32 , Article 1 , C .R. S . , as amended , and shall
be known as the "Tri-Area Ambulance District" .
Said District is described in exhibit "A"
The first Board of Directors of said District for one
year term shall consist of Cerise K. Bury , Joseph H . Rebel ,
and Robert Renner ; The first Board of Directors of said
District for three ( 3 ) year term shall consist of Sylvia Nicm ls ,
Anthony E. Onorato , Sr . , Minnette Paul , and Delia Robles .
The filing of corporate surety bonds by said directors
and by the Treasurer of said District is dispensed with and said
persons may file their personal bonds without sureties .
Said District shall become a governmental subdivision of
the State of Colorado and a body corporate with all the powers of
a public corporation.
BE IT FURTHER ORDERED that the Board of Directors upon
their oath shall take such steps and proceeding as the needs
of the District required and that within 30 days hence the Clerk
of this Court shall transmit to the County Clerk and Recorder of
Weld County , Colorado , a copy of this Order and Decree for filinc
in their respective offices , and said District shall be and is
identified for administrative purposes as the Tri-Area Ambulance
District .

67 West of the 6th Principal Meridian, Weld County, Colorado; thence
South along the East line of Section 7 to the Southeast corner of said
