Betty Le ,� rogram Supervisor D to Colors o Rehabilitation Services 8AMta ""tegory II i..:quest for Proposal Page 1 of 10 ABSTRACT The Weld County Adult Remediation Initiative for the Disabled (Initiative) has been developed through the Weld County Division of Human Resources' Employment Services of Weld County (Employment Services) in conjunction with Centennial Developmental Services Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center. - Employment Services delivers major employment and training programs which include Job Training Partnership Act, Job Services, and Welfare Diversion. Employment Services is the Weld County Service Delivery Area. - Centennial Developmental Services, Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center provide support services to adu'Its with disabilities. 1. Project's Goals and Objectives It is the goal of Employment Services, Centennial Developmental Services Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center to develop a coordinated delivery system of services directed toward assisting Weld County employers, and placing and retaining adults with disabilities who are eighteen (18) years of age and older in jobs. Coordination will include: a. Increasing the number of job openings for individuals with disabilities by 50 received from employers from urban and rural Weld County as a result of an efficient and coordinated employer relations program b. Placing 18 individuals with disabilities in unsubsidized part-time and full-time employment at an average wage of $4.00 per hour c. Retaining the adult with disabilities in unsubsidized employment for a minimum period of three months 2. Number of Clients to be Served The Initiative will provide employment and training services to 45 adults with disabilities who are eighteen (18) years of age and older. 3. Geographic Area The geographic area will be Weld County, including Greeley and the surrounding rural areas. 4. Total Dollars Requested The total requested for the Initiative is $50,000. 5. Characteristics of Clients to Be Served The Initiative will target adults eighteen years of age and older with diagnosable disabilities. p p� 890082, Category " Request ..r Proposal Page 2 of 10 PROPOSAL NARRATIVE 1. Statement of Need Weld County covers an area of 4,004 square miles and encompasses 28 incorporated towns. Greeley's population of 60,699 is approximately 43% of the county's total population (139,000) , with most of the remaining population (57%) within rural Weld County. This Initiative seeks to serve employers and participants from Greeley and rural Weld County. The disabled population of Weld County is in need of counseling, medical and situational assessments, and vocational training and/or placement in a suitable vocational area. The primary barriers to employment and independence which prevent this group from reaching full potential are: a. unwillingness of employers to view the individual beyond apparent disability b. inaccessibility of work sites c. transportation needs for varying work hours d. soft labor market (limited opportunities for meaningful work) e. inability to compete with adults who have no diagnosable disabilities, with an additional burden of having to do so in an area of high unemployment Weld County has an estimated 13,900 population with disabilities. This figure is from the most current data base information from Centennial Developmental Services , Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center. Of this 13,900, an estimated 200 individuals with disabilities require supported employment services without which employment would not be maintained. Given Weld County's current high rate of unemployment, community coordination is essential . At present there are several agencies that offer a variety of services to individuals with disabilities. Coordination of these services, plus a specifically designed employer relations plan, would provide an infinitely more efficient process by which the needs of this targeted group could be met. Implementation of the project would also initiate and ensure on-going coordination of services between local agencies and decrease the possibility of duplication. It is anticipated that the cross-training and collocation of staff will result in increased sophistication in the delivery of employment, training, and support services. 2. Goals and Objectives/Scope of Work It is the goal of Employment Services, Centennial Developmental Services Inc. ., Colorado Rehabilitation Services, and Schaefer Rehabilitation Center to develop a coordinated delivery system of services directed toward assisting Weld County employers, and 890082 Category' Request 1,.r Proposal Page 3 of 10 placing and retaining adults with disabilities into jobs. a. Increase the number of job openings by 50 received from employers in urban and rural Weld County as a result of an efficient and coordinated employer relations programs - procure, through Centennial Developmental Services, Inc. , staff support to perform job marketing functions with employers throughout Weld County - develop and pursue an aggressive employer relations plan which would promote the identification and listing of job opportunities and employer needs with Employment Services - develop work stations or positions which offer the applicant the opportunity of working with a majority of adults without disabilities b. Place 18 adults with disabilities in unsubsidized part-time and full-time employment at an average wage of $4.00 per hour - develop a coordinated service delivery system by which the employment needs of the adult with disabilities may be met on an on-going basis - develop a referral system by which agencies offering services to adults with disabilities may refer clients for possible placement - develop five (5) on-the-job training contracts in an integrated environment in which the applicant can be placed - enroll forty-five (45) disabled adult participants, eighteen (18) years of age and older - place eighteen (18) adults with disabilities into full- or part-time employment with special emphasis on job retention c. Retain the person with disabilities in unsubsidized employment for a minimum period of three months - provide easily accessible support services to the participant while he/she is at work - provide supported employment services to meet the needs of both the employer and the participant 3. Scope of Work/Methodology The scope of work for the Initiative is to be for the period of March 6, 1989 through June 30, 1990. The majority of activities wi' l occur in rural Weld County and in Greeley. . Pre-Grant Award Phase (January 23, 1989 through March 6, 1989) To accomplish the tasks outlined in the goals and objectives of the program, the following preparatory activities are being, or will be, accomplished: - initiate a process through which employers who are interested in working with adults with disabilities may be identified by February 10, 1989 - develop working agreements (non-financial ) between Employment Services and Centennial Developmental Services, Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation 890082 Category`'" Request i „r Proposal Page 4 of 10 Center that will identify coordination and networking activities on an on-going basis, by March 31, 1989 - set up a system by which tracking can be done on all participants, by April 23, 1989 - begin networking process by which referrals to and from appropriate agencies can be made, by February 28, 1989 - determine through consultation those initial employers to which applicants can be referred, by February 28, 1989 - identify the array of incentives directed toward the employers as a result of coordination activities and OJTs provided in the Initiative, by February 28, 1989 . Grant Awarc. Phase (March 6, 1989 through June 30, 1990) The following steps will be taken at the onset and during the program year to ensure that the goals and objectives of the program are met: - network with existing local agencies for the purposes of meeting client needs and avoiding duplication of services, by June 30. 1990 - meet quarterly with Employment Services, Centennial Developmental Services, Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center to determine the progress of the Initiative, by May 31, 1989; August 31, 1989; November 30, 1989; February 28, 1990; and June 30, 1990 - report quarterly to the Adult Activities Committee of the Weld County Private Industry Council which will act as an employer advisory council to the Initiative - purchase qualified and professional expertise from Centennial Developmental Services, Inc. and Schaefer Rehabilitation Center for the purpose of implementation and provision of service!: to adults with disabilities, by March 31, 1989 - assess quarterly the numbers of women and minorities who are referred to and participating in the Initiative - meet with targeted employers, and initiate an effective employers relations plan to meet specific employment needs of targeted population as well as to the employer, by May 31, 1989 through June 30, 1990 - determine, in conjunction with Centennial Developmental Services, Inc. , Colorado Rehabilitation Services and Schaefer Rehabilitation Center, those services most beneficial to the applicant/participant, by May 31, 1990 and through the grant award phase - complete status reports to provide information regarding activities and progress to the goals of the program - provide required JTPA adult information to the Governor's Job Training Office . Post Grant Award (July 1, 1990 through September 28, 1990) Final review ana tabulation will take place to determine and rate the Initiative by September 28, 1990. Points of review will include: - statistical and interview information from employers and participants 890082 Category "' T Request i .,r Proposal Page 5 of - analysis information from the agencies involved in the Initiative - review of achievements to goals and objectives of the Initiative - outcomes of the Initiatives impact in rural Weld County compared to Greeley and coordination and networking - interviews with employers and participants to assess the strengths and weaknesses of the Initiative, by September 28, 1990 - modification, if appropriate, of on-going working agreements established with the agencies participating in the Initiative, by September 28, 1990 - a final review of the Initiative from the agencies involved in the Initiative, by September 28, 1990 - determining if additional services are necessary, i .e. , expanded transportation services for rural participants 4. Cooperation and Coordination Strategies Major coordination will occur among Employment Services, Centennial Developmental Services, Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center. Employment Services shall be responsible for: - managing an administering the Initiative - developing the employer relations program - assigning employer accounts to Centennial Developmental Services, Inc. 's staff persons - recruiting appropriate job openings - evaluating the Initiative - determining eligibility of program participants using JTPA guidelines and definitions - monitoring and placing adult participants in employment Centennial Develo?mental Services, Inc. , shall be responsible for: - providing qualified staff workers to assume caseload of employers with whom to develop employer relations plans in regard to client placement - development of specific Employment Service training plans for participants, according to the needs of the employer Schaefer Rehabilitation Center and Centennial Developmental Services, Inc. , shall , through coordination, be responsible for: - obtaining referrals from appropriate agencies - monitoring of client activities and performance at work site - providing supported employment services, as required by employers - providing supported employment coaches - referral and recruitment of clients, including minorities and women with disabilities - providing client performance evaluations and follow-up according to the guidelines established by Schaefer Rehabilitation Center and Centennial Developmental Services, Inc. - developing a client training plan (cluster occupation areas) - monitoring client activities and performance - providing follow-up of client performance at the work site 890082 Category Request tor Proposal Page 6 of 10 Colorado Rehabilitation Services shall be responsible for: - accepting referrals from appropriate agencies - determining eligibility for services from Colorado Rehabilitation Services based on the evaluation of physical or mental disability which creates a functional limitation and subsequent employment barriers for the individual - referring appropriate clients to Employment Services - developing individualized written rehabilitation programs for clients - coordinating payment for client services with other agencies - monitoring Colorado Rehabilitation Services client activity and performance - providing follow-up and job retention counseling 5. Implementation/Evaluation The Initiative is designed to provide a coordinated system by which employment services can be provided to adults with disabilities. Resources thrcugh this grant will serve to initiate the project. All agencies Participating in this project will continue the services outlined in this Initiative. These services will be identified through on-going, working agreements developed with the participating agencies. Coordination among Employment Services, Centennial Developmental Services, Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center will combine existing services and develop new services to create a concurrent delivery of services directed toward meeting the current and future employment needs of adults with disabilities. To determine the effectiveness and appropriateness of the efforts under this program, Employment Services shall evaluate this project according to the goals and measurable objectives established for the program. a. Evaluation will seek to determine if Employment Services, Centennial Developmental Services Inc. , Colorado Rehabilitation Services, and Schaefer Rehabilitation Center, developed a coordinated delivery system of services directed toward assisting Weld County employers and placing and retaining adults with disabilities in jobs by: - increasing the number of job openings for individuals with disabilities by 50 received from employers from urban and rural Weld County - placing 18 individuals with disabilities in unsubsidized part-tme and full-time employment at an average wage of $4.00 per hour - retaining the adult with disabilities in unsubsidized employment for a minimum period of three months b. Methods to be applied in evaluation process are: - review quarterly and final reports from agencies involved in the Initiative 890082 Category-" Request . ,r Proposal Page 7 of 10 - review of prescriptive employer plans in regard to applicant and employer needs - review of supported employment plans in regard to meeting participant's training needs - select a random sample of participating employers for interviewing purposes to determine effectiveness of client training and performance - select a random sample of participants for interviewing purposes to determine the degree of satisfaction - analyze various reports regarding participant status Evaluation information will be provided for review to the Weld County Private Industry Council , Board of Weld County Commissioners, and the Supported Employment Consortium on a quarterly basis. 6. Administrative Capability The Initiative will be administered by Employment Services with intensive coordination between Employment Services, Centennial Developmental Services, Inc. , Schaefer Rehabilitation Center, and Colorado Rehabilitation Services. a. Staffing Flan Employment. Services will administer and manage the Initiative. Additionally, Employment Services will provide in-kind staffing for intake, eligibility determination/verification, placement, and other employment and training functions. Employment Services will purchase a total of 1,385 staff hours from two staff from Centennial Developmental Services, Inc. to perform job marketing and employer relations functions of Employment: Services. Employment Services will purchase a total of 2,800 staff hours for a pool of job coach services from Schaefer Rehabilitation Center and Centennial Developmental Services, Inc. Colorado Rehabilitation Services will provide, at no cost to this Initiative, through a working agreement, coordination activities; for job coach resources and will broker services of the Colorado Rehabilitation Services. b. Management and Administrative Plan - Managenent and Administration Plan, regarding accounting system, fiscal reporting policy, participation tracking system, bonding issues and assurances, is described in Attachment I . - Organizational Structure: The organizational structure of the Employment Services is described in Attachment II . 890082 Category Request i .r Proposal Page 8 of 10 - Monitoring and Evaluation System: A monitoring and evaluation process has been developed and will involve: 1) review of quarterly progress reports of client placement and participation 2) designation of appropriate committees to monitor and evaluate areas such as program implementation, comprehensive planning, and review of purchase of service agreements 3) monitoring work experience sites regularly 4) internal eligibility verification process - Personnel System: The Weld County Board of County Commissioners has adopted Personnel Policies and Procedures. The Personnel Policies and Procedures are administered in accordance with Equal Opportunity laws, the revised Merit System Standard, and the Weld Home Rule Charter. The personnel system has been certified under the Merit System of Personnel Administration from the Office of Personnel Management. - Grievarce/Complaints System: Complaints procedures are propro ivded to all participants upon enrollment into JTPA. See Attachment III . All complainants shall be notified of the process for sending complaints to Governor's Job Training Office, if complaints are not resolved at local level . Procedures apply for the following: 1) Non-Criminal Grievance/Complaints Which Do Not Allege Discrimination 2) Grievance/Complaints Which Allege Discrimination - Affirmative Action: Employment Services abides by an Affirmative Action Plan set in place by Weld County. A copy of the resolution is Attachment IV. - Board of Director Membership: The Board of Directors is the Board of Weld County Commissioners. The Weld County Private Industry Council is the advisory committee to the Board of Weld County Commissioners and advises the Board of Weld County Commissioner concerning the programs and services of the Employment Services. See Attachment V for a description of the Weld County Private Industry Council . 7. Past Performance Employment Services has provided services to adults and youth with disabilities through Job Services and Job Training Partnership Act Title II-A activities. See Attachment VI for a description of Employment Services' past performance. 6S 062 Category ' Request l .,r Proposal Page 9 of 10 Additionally, Employment Services has expanded its services to individuals with disabilities by: - installing telephone equipment for the hearing impaired - contracting with the Northern Colorado Center on Deafness for translating services for the deaf - transporting the disabled from throughout rural Weld County Employment Services is the major agency in Weld County to provide comprehensive services to employers through incentives of Job Services, Job Training Partnership Act, Welfare Diversion, Unemployment Insurance Internship, and other resources. For the year ending June 30, 1988, Employment Services' Job Services component was rated number one in Colorado for cost per placement and number two in Colorado for the number of placements obtained for job applicants. Employment Services met or exceeded its goals under Title II-A for the year ending June 30, 1988. 8. Narrative Budget Information Summary The budget for the Initiative is: Descriotion Budget Request a. Administration @ 5% - audit, space costs, supplies and management for the project $ 2,500 b. Training @ 95% $42,500 - Or-the-Job Training for 6 months for 5 individuals @ $2,349 each or $11,745 - Purchase of Service Agreement with Centennial Developmental Services, Inc.: staff members for a total of 1,385 hours @ $8.75 per hour or $12,000 - Purchase of Services Agreement with Centennial Developmental Services, Inc.: job coach hours at 2,800 @ $6.70 per hour or $18,755 TOTAL $50,000 The match budget for the Initiative will total $50,000 as described below: a. Centennial Developmental Services will provide a total of $43,488 in match resources (in-kind) : - 20% of Director of Employment Services for coordination purposes: $5 700 - 50% of Employment Specialist II for marketing: $12,250 - 1 hours at $6.70 per hour for job coach services es: 17:500 b. See Attachment VIII: Match Budget summary Form 9. Project Innovation The applicability of this Initiative to non-disabled populations is exciting, especially for placement and job retention. Whether through counseling, training, mediation, procurement of necessary services, transportation services are provided until the employer and employee both agree that the placement agency's services are no longer needed. The incentive for the employer becomes more than financial . By guaranteeing the quality of the employee (client) , the agency makes the same commitment the employer makes to his or her customers. In Weld County, the preliminary applications of this concept are taking place under this Initiative for adults with disabilities. It is anticipated that the pooling, cross-training, and collocation of staff will result in increased sophistication in the delivery of employment, training, and support services. 10.. Rural Delivery System Weld County Employment Services currently offers services regarding referral , placement, and transportation in rural Weld County. With the implementation of the Initiative, expansion of its services may be made. Until this Initiative, it has been cost prohibitive for Centennial Developmental Services, Inc. , and Schaefer Rehabilitation Center to do supported employment in rural Weld County. Better access to rural employers should lead to more opportunities for rural disabled citizens near their homes. In addition, outreach and new incentives to rural employers through prescriptive employer plans will create new opportunities for both the employer and the participant. 890082 Catagory II Request for Proposal AtttachmentI page I of 6 JTPA CONTRACTOR ASSURANCE STATEMENT A. LcguLALLLitnlity ' The Contractor assures and guarantees that it possesses the legal author- ity to enter into this Contract . The person or persons signing and executing this Contract on behalf of the Cant carter do hereby warrant and guarantee that they have been fully authorized by the Contractor to execute this Contract on behalf of the Contractor. B. financial Reporting Requirements The Contractor shall use detailed financial reporting procedures including budgeting, invoicing, financial procedures and budget revisions and modifications described in the "Financial Reporting Procedures" which shall be provided to the Contractor. Unless otherwise identified in the Statement of Work, payment is based on the reimbursement of actual costs incurred. C. Mtnngement Activities and Record Maintenance In addition to any requirements imposed elsewhere in this Contract, the Contractor shall ensure that sufficient, auditable and otherwise adequate records that will provide accurate, current, separate and complete disclosure of the status of the funds received under the Contract are maintained for three (3) years or the completion and resolution of an audit. Such records shall be sufficient to allow the U.S. Department of Labor, firms conducting audits of JTPA funds and CJTO to audit and monitor the Contractor. 090082 Catagory II Request for Proposal Attachment I Page 2 of 6 E. Suhrontrartinv The Contractor shall not subcontract the performance of any part of its duties under this Contract except in accordance with the terms of :his Contract or with the prior written consent of the CCCOES approving the subrectpient : F. Property ianagement The Contractor must submit a written request to CCCOES prior to the purchase with JTPA funds of any equipment with a unit cost of five hundred dollars ($500) or more. CCCOES will seek approval from G_TO. Upon receipt - of CJTO' s written approval, the Contractor may proceed with the purchase, and submit a New Equipment Notification Form to the CCCOES. The GJTO shall retain title to all equipment, in excess of five hundred dollars ($500) , purchased by the Contractor, and, therefore, r:ior written approval is needed before disposition of equipment purchase.: by the Contractor with funds from GJTO. The Contractor shall comply the GJTO property management procedures and all relevant JTPA letters. The Contractor ensures that it will maintain proper inventory control over all equipment purchased with JTPA funds. G. programmatic Modification-, The Contractor shall follow the revision procedures established by CCCOES. The Contractor must submit a written request to the CCCOES and obtain prior written approval for changes to the Statement of Work or objective of the Contract. H. Assurarulea 1. The Contractor assures that it will fully comply with the Job Training Partnership Act (JTPA) , regulations promulgated, and all other federal regulations issued pursuant to the Act. 2. The Contractor assures that in operating programs funded under the Act, it agrees to comply with all GJTO issuances and directives, such as JTPA letters that are distributed. 3 . The Contractor agrees to abide by the provisions of Section 144 pf the JTPA which concerns grievance procedures. Further, the Contrac- tor agrees to follow Federal Regulations governing grievance reso- lution of all complaints, including those • based . on handicap, but excluding complaints alleging discrimination. All discrimination complaints except those based on handicap must be filed initially at the U.S. Office for Civil Rights. Finally, the Contractor also agrees to follow all pertinent Governor's policy issuances concerning grievance procedures. 2 590082 Category II Request for Proposal Attachment I Page 3 of 6 4. The Contractor assures and certifies that in administering programs under JTPA: a . It will comply with the provisions of the Unitorm Relocation Assistance and Real Property Acquisition Act of 1970 (P.L. 91- 646) which requires fair and equitable treatment of persons displaced as a result of federal and federally-assistecci programs. b. It will comply with the provisions of the Hatch Act, where applicable, which limit the political activity of certain state and local government employees. c . It will comply with appropriate labor standards as set forth in Section 143 of the JTPA. (20 CFR 629.22 and 629. 33) d. It will comply with the Child Labor laws ; e. It will comply with the Safety Standards of Occupational Safety and Health Act (OSHA) ; f. It will comply with the Davis Bacon Act ; g. It will comply with the Wagner-Peyser Act; h. It will comply with Part C of Title IV of the Social Security Act; i. It will comply with the Military Selective Services Act; ^q j . It will comply with Section 665, Title 18 United States Code; i;Fraud, Abuse, Malfeasance) k. It will comply with the Fair Labor 'Standards Act of 1938; 1. Box 1805; 1516 Hospital Road Greeley, Colorado 80632 (303) 353-0540 This procedure shall be followed for complaints involving the Job Training Partnership Act, based upon a complaint which alleges that a person was discriminated against because of handicap. A handicap discrimination complaint may be raised with the Department Head of the program involved at any time. Th< Department Head will make every attempt to resolve the problem. The person raising the problem will be advised of the procedures to follow if the person wants to file a complaint. The Complainant will follow the following procedures in the filing of a complaint: Step 1: The Complainant will file the complaint in writing. The Equal Employment Opportunities/Affirmative Actior (EEO/AA) Officer will assist the Complainant in completing the form "Notice of Formal Complaint." The filing of a JTPA handicap discriminatory complaint shall be made within one-hundred eighty (180) calendar days of the alleged occurrence, unless the time for filing has been extended by the Assistant Secretary of the United States Department of Labor. The EEO/AA Officer will inform the Colorado Governor's Job Traininc Office of the formal filing of the handicap discrimination complaint. The Complaint shall immediately be sent to the attention of the Personnel/Client Board by the EEO/AA Officer. Step 2: The Personnel/Client Board must conduct an impartial hearing to review the handicap discrimination complaint within thirty (30) days of its filing. The impartial Personnel/Client Board will direct the EEO/AA Officer in the preparation and review of a complete file on the case prior to the hearing. The Personnel/Cliert Board will : a. Direct parties to appear at the hearing; provide notice of the date, time, and place of the hearing at least five (5) calendar days prior to the hearing; the manner in which it will be conducted; and the stipulated issues to be heard. b. Advise all parties that they are able to be represented by counsel or the representative of their choice; they have the opportunity to bring witnesses and documentary evidence. c. Conduct the hearing in accordance with the attached procedures. d. Question witnesses and parties. e. Consider and evaluate the facts, evidence, and arguments to determine credibility. f. Render a decision. The Personnel/Cliert Board will give a written recommended decision setting forth findings of fact and give the reasons for the decision to the Complainant, the Department Head, and the EEO/AA Officer. The written recommended decision must be presented within forty-five (45) calendar days of the date the Complainant filed his or her handicap discrimination complaint with the EEO/AA Officer. The EEO/AA Officer must then mail the recommended decision to the Governor of the State of Colorado, and such mailing must be postmarked no later than the forty-fifth (45th) day of the filing of the complaint. Step 3: The Governor of the State of Colorado will issue a final decision within sixty (60) days of the date that the Complaint filed his or her handicap discrimination complaint with the EEO/AA Officer. The Governor's final decision will be in writing and will be sent via certified mail to the Complainant and to the Depart- ment Head, the EEO/AA Officer, and the Personnel/Client Board. Step 4: The Complainant may appeal the decision of the Governor of the State of Colorado to the Assistant Secretary of the United States Department of Labor within thirty (30) calendar days of the date of the Governor's decision. The EEO/AA Officer shall notify the Complainant, in writing, of the appeal process and the address for appeals. NOTE: The identity of all persons who have furnished information relating to a complaint or assisting in the obtaining of facts, shall remain in confidence to the extent possible consistent with a fair determination of the issues.
