RE: APPROVE AMENDMENT NO. V TO THE HEALTH PLAN DOCUMENT OF THE
WELD COUNTY EMPLOYEE BENEFIT FUND AND AUTHORIZE CHAIRMAN TO
SIGN SAME
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment No. V to
the Health Plan Document of the Weld County Employee Benefit Fund,
and
WHEREAS, said Amendment adds an employee hospital bill audit
benefit, payable at 50% of the discovered error to a maximum of
$250 per admission, and
WHEREAS, the Board has reviewed said Amendment No. V and
deems it advisable to approve same, a copy being attached hereto
and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. V to
the Health Plan Document of the Weld County Employee Benefit Fund
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Amendment No. V.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 25th day of
March, A.D. , 1985 .

AMENDMENT NO. V
to the Health Plan Document of
THE WELD COUNTY EMPLOYEE BENEFIT PLAN
The Health Plan Document of The Weld County Employee Benefit Plan is hereby
amended, effective March 1, 1985, as follows:
The following paragraphs shall be added in their entireties to the Master Plan
Document:
Employee Hospital Bill Audit 50% of the discovered error to
Benefit maximum of $250 per admission
EMPLOYEE HOSPITAL BILL AUDIT BENEFIT
Benefit Provision
If a Covered Person should discover an error in a Hospital billing that results
in a refund to the Plan, the Covered Employee shall be entitled to an amount
equal to fifty (50%) percent of the refund of a maximum of $250 per admission.
This benefit will not extend to an error in billing found and corrected by the
Hospital and/or the Contract Administrator. Benefits shall not be distributed
until after the Plan has received the refund.
IT IS AGREED BY WELD COUNTY that the provisions contained in the Plan Document
and Amendment No. V thereto are acceptable and will be the basis for the admi-
nistration of said Employer's Employee Benefit Program described herein.
