Purchase of Services Agreement between Weld County Human esources
Subject: and Centennial Developmental Services, Inc.
Enclosed for Board approval is a Purchase of Services Agreement
between Weld County Division of Human Resources (HRD) and
Centennial Developmental Services, Inc. (CDSI) Weld County
Division of Human Resources will provide CDSI transportation
services for their clients. CDSI will reimburse HRD for actual
per mile costs of gas, oil , repair and maintenance salaries,
vehicle replacement and administrative overhead. HRD and CDSI
will review the rates quarterly and adjust if necessary.

URCHASE OF SERVICES AGREEMENT 6Q-}VLl
THIS AGREEMENT, made and entered into this 21 st day of August, 1987, by and
between the Weld County Division of Human Resources , ereinafter referred to as
"HRD" and Centennial Developmental Services, Inc. , hereinafter referred to as
"CDSI".
WITNESSETH:
WHEREAS, HRD administers a door-to-door transportation system for the low
income, elderly, and handicapped, and
WHEREAS, CDSI is & non-profit community incorporated board which provides
services for the developmentally disabled, and
WHEREAS, CDSI is desirous of entering into an agreement to purchase all of
their program's transportation requirements from HRD.
NOW THEREFORE, it is mutually agreed as follows:
1. HRD agrees to maintain a flexible transportation system with the capacity
to meet the transportation requirements of the CDSI Program.
2. HRD agrees that mileage reimbursement rates will be determined from actual
per mile costs for administrative overhead, gas and oil , repair and
maintenance, salaries , and vehicle replacement.
3. HRD and CDSI agree to review quarterly actual transportation costs,
reimbursement rates, and total miles of transportation and make
appropriate adjustments prior to billing.
4. HRD agrees to bill CDSI within 15 days of the closing date of each monthly
financial period.
5. CDSI agrees to reimburse HRD monthly and within 30 days of the receipt of
each billing.
6. It is agreed between the parties that either party hereto has the right to
terminate this Agreement upon thirty (30) days written notice given to the
other party.
7. This Agreement may be amended with the written consent of both parties.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the
date first written above.
