Enclosed for Board approval is the Weld County Division of Human
Resources ' 1989 Emergency Community Services Homeless (ECSH)
application.
The grant application proposes to sub-contract with Weld Information and
Referral Services to continue the current case management program for
the homeless.
We are requesting approval of a funding level of $16,800.00 for the
period July 1, 1989 through June 30, 1990.

B. PROBLEM/NEED DESCRIPTION
In order to better serve the homeless population of Weld County,
Weld Information and Referral Service (WIRS) implemented a Homeless
Case Management Program to serve the homeless population in 1988.
WIRS would continue to provide this service if funded. The program
would be expanded to include more follow-up of clients to better identify
continuing needs and evaluation of the system. It is estimated that 8,000
individuals will face homelessness in 1989. For Weld County, 67% of those
are families with dependents. Currently, the program offers case
management for 20 families per month. The program has 35 families pending,
to participate in the program. The program incorporated an agreement with
15 Human Services providers located throughout Weld County to coordinate
the process of alleviating the homeless issue in Weld County, The program
has a 60% success rate for families completing the program. The case
management system effectively and efficiently links the appropriate resources
for the homeless individuals thereby shortening their stay in shelters
and returning them to self-sufficiency.

C. PROJECT IMPACT
It is estimated that 8,000 indivudals are homeless a year in Weld County.
Of this number, at least half need more than just housing. The case
management system efficiently links the appropriate resources
for homeless individuals.
D. DESCRIPTION OF LOCAL GOVERNMENT COORDINATION

Short term emergency shelter services in Weld County are provided by
the Guadalupe Center, Salvation Army, and D.A.V. Outreach Center.
Longer term shelter, 30 to 45 days, is provided by Transitional House and
A Woman' s Place with supporting services designed to move clients to a
position of self-sufficiency as quickly as possible. All of these agencies
are engaged in cooperative casework managment activities and are networking
daily with each other and 17 other providers of social services, representing
both local government and private agencies. The range of services
available include employment, food, clothing, medical and dental services,
day care, recreation, and mental health counseling. This proposal is
a continuation of the Weld County Housing Coalition goals established last
year. The Coalition is comprised of approximately 30 local service
providers. The purpose of the coalition has been to share information,
coordinate services, pool resources where appropriate and plan for the
future housing needs of our community. The entire coalition is supportive
of the Case Management Program and participate in the program.

ECSH RESOLUTION & CERTIFICATION
The County of Weld , which is applying to the
State of Colorado for funding through the Emergency Community Service
Homeless (ECSH) Grant program from the U.S. Department of Health &
Human Services, hereby assures and certifies that:
- the amounts awarded under the ECSH program will be used only for
the following purposes;
Expansion of comprehensive services to homeless individuals
to provide follow-up and long-term services to enable
homeless individuals to make the transition out of poverty,
Provision of assistance in obtaining social and maintenance
services and income support services for homeless
individuals,
Promotion of private sector and other assistance to homeless
individuals,
Services for the prevention of homelessness,
- no amount received under this program will be used to supplant
other programs for homeless individuals administered by the
grantee;
- political activities will be prohibited in accordance with •
subsection 675 (e) of P.L. 97-35 as amended (the Community
Services Block Grant Act) ,
- there will be coordination of activities among all participating
local governments and non-profit organizations funded by the
State,
- the contractor or its subcontractors shall have been providing
critically urgent homeless services as of January 1, 1987, and
- no more than 25% of the grantees funds will be used for homeless
prevention.
- the contractor and/or its subgrantees shall develop fiscal
control and fund accounting procedures necessary to assure the
proper disbursal of and accounting for federal funds.
The County ensures the cooperation of its staff, the cooperation of
its subcontractors, and the availability of all records to the
Department of Local Affairs and the Office of the Secretary of Health
& Human Services or his/her designee charged with monitoring or
investigating the use of the funds provided.
