Enclosed for Board approval is a Purchase of Services Agreement between
Weld County Division of Human Resources' Area Agency on Aging and
Practorcare, Inc. , for Training and Technical Assistance support for the
Senior Nutrition staff to learn to utilize the Nutriplanner - 4000 soft-
ware. This software was recently' purchased for menu planning and
analysis for the Weld County Senior Nutrition Program.
Under the terms of this agreement, the Area Agency will reimburse
Practorcare $700.00 to provide two-four hour training sessions plus
Technical Assistance support for one year.
The services of this agreement will remain in effect un January 20,
1990.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this 1st day of February,
1989, by and between Weld County Division of Human Resources" Area
Agency on Aging, hereinafter referred to as "AAA", and Practorcare, Inc. ,
hereinafter referred to as "Practorcare".
WITNESSETH
WHEREAS, AAA has purchased the Nutriplanner--4000 software package
for menu planning and analysis for the Weld County Senior Nutrition
Program, and
WHEREAS, AAA is desirous of training and support that will enable
Senior Nutrition staff to utilize the Nutriplanner software for the
maximum benefit of the program, and
WHEREAS, Practorcare has the services of a registered dietitian to
provide such training and support,
NOW THEREFORE, it is mutually agreed:
1. Practorcare will provide two four-hour sessions of on-site
training to be conducted in separate visits by a registered dietitian.
Such training will consist of software installation, an overview of all
program features, practice exercises, practical applications, and a
question and answer session.
2. Practorcare, in addition to the on-site training, will provide
the following support services which will be covered for one year under
this agreement.
SOFTWARE MAINTENANCE: Changes in food data, nutrient values ,
diabetic exchanges, RDA's, equations, and other database items.
They are released as research recommends and in a timely manner.
SOFTWARE ENHANCEMENTS: New program features added to existing
software. They are released as soon as development and testing is
completed.
CUSTDMER SUPPORT: Available both from the Denver based Regional
Sales Manager and from home office based nutrition and food service
specialists. Either office may be called when necessary and the
"800" number phone line support may be used when callinc the home
office.
3. The AAA will reimburse Practorcare $700.00 (plus $0.22 per mile
for two round trips from Denver to Greeley.

4. The services of this agreement will remain in effect from January 20, 1989
through January 20, 1990.
