The agreements are between the Employment Opportunities Delivery System (EODS) Summer Youth Program and the following contractors: Contractors Agreement Amount 1. Weld County Community Center Foundation $5,280 2. North Colorado Medical Center $2,640 3. Lawrence E. Parker $ 450 4. Ken Magruder/Waenetta Elliott $ 900 5. Richard Mitchell $1 ,350 The contractors will be conducting special employment and training workshops for all Summer Youth Program enrollees. The workshops will meet once a week for six weeks. Some of the contractors are conducting more than one workshop during the course of a week. If you have any questions regarding the agreements or the special workshops, please contact me. 850899 PURCHASE OF SERVICES THIS AGREEMENT, made and entered into this 1st day of July, 1985, by and between the Weld County Division of Human Resources' Employment Opportunities Delivery System, hereinafter referred to as "EODS" , and the Weld County Community Center Foundation, hereinafter referred to as "WCCCF" , WITNESSETH: WHEREAS, EODS delivers employment and training services to youth throughout Weld County; and WHEREAS, EODS is interested in providing education and training programs for the Youth Employment and Training Programs; and WHEREAS, WCCCF provides specialized educational programs for eligible clientele; NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree to as follows: 1. Definitions/Assumptions: A. A workshop is composed of two (2) sessions. Each session offers a series of six classes. Each class is based on a different subject. B. Each session is four (4) hours in duration and convenes once a week. C. Each session is composed of groups of no more than twenty-five (25) youth participants. D. Two (2) workshops (4 sessions) will be conducted in both Greeley and/or Fort Lupton depending upon demand. E. A minimum total of twenty-four (24) classes will be held. 2. Responsibilities of WCCCF are: A. To plan and present a minimum of two (2) workshops on a weekly basis for the EODS Youth Employment and Training Program. B. To present the workshops between the time period of July 8, 1985 - August 16, 1985. C. To provide all workshop materials. D. To provide EODS with participant evaluations following each workshop. 3. Responsibilities of EODS are: A. To arrange for classroom space for all workshops and to provide notification of the location and date/time to WCCCF and participants. B. To arrange for transportation of the youth employees to and from the workshops. C. To schedule one (1) youth program supervisor to attend and supervise the youth during the classes. D. To provide audiovisual equipment for the workshops whenever possible. E. To take attendance at each workshop session. 4. Operational Time Frame: This agreement will be in effect from July 8, 1985, through August 16, 1985. 5. Reimbursement: In consideration of the services provided by WCCCF, EODS will reimburse WCCCF at the rate $220.00 per session for a maximum reimbursement of $5,280 ($220.00 per session x 4 sessions per week x 6 weeks = $5,280). Reimbursement will be made within thirty days of the completion of the workshops and with the stipulation that all participant evaluations have been received by EODS. Additional charges will be assessed as follows: A. Whenever the maximum enrollment level of 25 is exceeded, an additional $10.00 will be charged for each individual over the maximum enrollment level . B. In the event that WCCCF audiovisual equipment must be used, charges will not exceed $35.00 per class . These charges are in addition to the $220.00 being charged for each session. The total reimbursement for audiovisual equipment cannot exceed $840.00 ($35.00 x 24 classes). 6. Amendment of Agreement: EODS and WCCCF consent that this agreement may be amended at any time with the mutual written consent of both parties. 7. Termination of The Agreement: EODS and WCCCF consent that this agreement may be terminated upon thirty (30) days written notice by either party. Furthermore, this agreement and all contracts associated with this agreement will be terminated immediately if the funding provided by the Job Training Partnership Act (JTPA) is withdrawn by the Governor' s Job Training Office or the U.S. Department of Labor. RD ai BX• frja�✓n e,, De uty County Clerk AGREEMENT This agreement is affected this 28th day of June, 1985 by and between North Colorado Medical Center (referred to herein as the "Medical Center") and the Weld County Division of Human Resources (referred to herein as the "H.R.D. ") .. Whereas the Medical Center provides health promotion programs for Weld County businesses and agencies and, Whereas the H.R.D. is interested in providing educational programs for the Youth Employment Program, The Education Department of the Medical Center will present educational programs at a site designated by the H.R.D. for 14 and 15 year old youth employees in an attempt to further the following goals: 1 . To promote good work habits and understanding of the working world. 2. To encourage informed decision making regarding physical , mental , and social health. 3. To encourage and promote essential life skills. It is mutually agreed: A. Responsibilities of the Medical Center 1 . To plan and present 4 to 6 four-hour workshops for the H.R.D. Youth Employment program. 2. To present these workshops in two sessions to groups of no more than 25 youth employees per session. 3. To present sessions between July 1 and August 16 on Tuesdays and Thursdays from 8:00 a.m. to 12:00 noon (with the exception of July 4) . 4. To provide workshop materials. 5. To provide the H.R.D. with participant evaluations following each workshop. B. Responsibilities of H.R.D. 1 . To arrange for classroom space for all workshops. 2. To arrange for transportation of youth employees to the workshops. 3. To schedule one youth program supervisor to attend and supervise youth during the sessions. 4. When possible, to provide audiovisual equipment for the workshops. AGREEMENT H.R.D. Page 2 C. Reimbursement for Services In consideration of the services provided by the Medical Center, the H.R.D. will pay $440 per workshop to North Colorado Medical Center within thirty days of the completion of the workshops. Additional charges will be assessed as follows: $10/person for each person over 25 persons/session In the event that North Colorado Medical Center audiovisual equipment must be used, charges not to exceed $35/session will be assessed. D. Terms of the Agreement This program will begin July 1 and end August 16, 1985. To present the workshops between the time period of July 8, 1985 - August 16, 1985. C. To provide all workshop materials. D. To provide EODS with participant evaluations following each workshop. 3. Responsibilities of EODS are: A. To arrange for classroom space for all workshops and to provide notification of the location and date/time to the Contractors and participants. B. To arrange for transportation of the youth employees to and from the workshop. C. To schedule one (1) youth program supervisor to attend and supervise the youth during the classes. D. To provide audiovisual equipment for the workshops whenever possible. E. To take attendance at each workshop session. 4. Reimbursement will be made within thirty days of the completion of the workshop and with the stipulation that all participant evaluations have been received by EODS. Additional charges will be assessed as follows: A. Whenever the maximum enrollment level of 25 is exceeded, an additional $10.00 will be charged for each individual over the maximum enrollment level . This amount is in addition to the normal charge of $75.00 per class and is based on attendance records maintained by EODS. E . Amendment of Agreement: EODS and the Contractor consent that this agreement may be amended at any time with the mutual written consent of both parties. 7. Termination of The Agreement: EODS and the Contractor consent that this agreement may be terminated upon thirty (30) days written notice by either party. Operational Time Frame: a This agreement will be in effect from July 8, 1985, through August 16, 1985. 5. Reimbursement: In consideration of the services provided by the Contractors , EODS will reimburse the Contractors at the rate $75.00 per class for a maximum reimbursement of $900.00 ($75.00 per class x 2 classes per week x 6 weeks = S900.00). Reimbursement will be made within thirty days of the completion of the workshop and with the stipulation that all participant evaluations have been received by EODS. A billing statement will be required. A seperate check will be processed for each individual contractor in accordance with the billing. Additional charges will be assessed as follows: A. Whenever the maximum enrollment level of 25 is exceeded, an additional 510.00 will be charged for each individual over the maximum enrollment level . This amount is in addition to the normal charge of $75.00 per class and is based on attendance records maintained by EODS. 6. Amendment of Agreement: EODS and the Contractors consent that this agreement may be amended at any time with the mutual written consent of both parties. 7. Termination of The Agreement: EODS and the Contractors consent that this agreement may be terminated upon thirty (30) days written notice by either party. Furthermore, this agreement and all contracts associated with this agreement will be terminated immediately if the funding provided by the Job Training Partnership Act (JTPA) is withdrawn by the Governor's Job Training Office or the U.S. Department of Labor.
