Subject: Amendment to Purchase of Service Agreement between
Migrant Head Start and Sunrise Community Health Center
Enlosed for Board approval is an amendment to the original
contract between Migrant Head Start and Sunrise Community
Health Center.
We are amending the original agreement by increasing the amount
of reimbursement for medical and dental services provided for
the Migrant children from $2,000.00 to $5,000.00

AMENDMENT TO PURCHASE OF SERVICES AGREEMENT
THIS AMENDMENT, made and entered into this 29th day of July, 1987,
by and between the Weld County Division of Human Resources' Migrant Head
Start Program, hereinafter referred to as "Head Start," and the Sunrise
Community Health Center, hereinafter referred to as "Health Center."
WITNESSETH
WHEREAS, Head Start and the Health Center are desirous of amending
the original agreement of May 4, 1987.
NOW THEREFORE, the following condition is mutually agreed upon:
The amount of the original agreement of $2,000.00 shall be
changed to $5,000.00.
IN WITNESS WHEREOF, the parties hereunto have caused this amendment
to de duly executed as of the day and year first hereinabove set forth.
