RE: APPROVE AMENDMENT NO. I TO MASTER PLAN DOCUMENT OF WELD
COUNTY EMPLOYEE BENEFIT PLAN AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS , the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS , the Board has been presented with Amendment No. I to
the Master Plan Document of the Weld County Employee Benefit Plan,
and
WHEREAS, said Amendment adds a paragraph concerning
Coinsurance and amends the provider definition of Semi-Private as
stated in the Amendment, a copy of which is attached hereto and
incorporated herein by reference, and
WHEREAS, after study and review, the Board deems it advisable
to approve said Amendment.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. I to
the Master Plan Document of the Weld County Employee Benefit Plan
be , and hereby is , approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Amendment.
The above and foregoing Resolution was, on motion duly made
and seconded , adopted by the following vote on the 18th day of
May, A.D. , 1988 , nunc pro tunc October 1 , 1987 .

AMENDMENT NO. I
to the Master Plan Document of
WELD COUNTY
The Master Plan Document of THE WELD COUNTY Employee Benefit Plan is hereby
amended, effective October 1, 1987, as follows:
Page 1 - The following paragraph is added in its entirety:
SUMMARY OF BENEFITS FOR EMPLOYEES AND DEPENDENTS
Coinsurance
For in-patient services provided by NORTH COLORADO MEDICAL CENTER
whose billing practice separates the nursing component of the room
and board fees and bills these services separately, in addition to
the room and board charge, the Plan will allow the UCR nursing
service fees as part of the room and board benefit, subject to all
existing Plan provisions.
Page 7 - The following paragraph is deleted in its entirety:
Provider Definitions
Semi-Private - A hospital room containing two (2) or more beds but
does not include the charge made by the hospital for Intensive
Care.
Page 7 - The following paragraph is added in its entirety:
Provider Definitions
Semi-Private - A hospital room containing two (2) or more beds but
does not include the charge made by the hospital for Intensive
care. For NORTH COLORADO MEDICAL CENTER who bills separately for
the nursing component of the room and board fees, the UCR nursing
services fees will be considered as part of the allowable semi-
private hospital room and board fees.
IT IS AGREED BY WELD COUNTY that the provisions contained in the Plan Document
and Amendment No. I thereto are acceptable and will be the basis for the
administration of said Employer's Medical Benefit Program described herein.
