RE: APPROVE MAMMOGRAPHY AGREEMENT BETWEEN WELD COUNTY AND NORTH
COLORADO MEDICAL CENTER AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement
between Weld County and North Colorado Medical Center regarding
mammography screening, and
WHEREAS, the terms and conditions are as stated in said
Agreement, a copy of which is attached hereto and incorporated
herein by reference, and
WHEREAS, after review, the Board deems it advisable to
approve said Agreement.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that the Agreement between
Weld County and North Colorado Medical Center be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Agreement.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 2nd day of
September, A.D. , 1987 .

MAMMOGRAPHY AGREEMENT
This document serves as a letter of agreement between Weld County
and North Colorado Medical Center regarding mammography screening.
Terms of the agreement follow:
Responsibilities of North Colorado Medical Center
1. To provide mammography screening for employees of
Weld County.
2. To provide screening during the month of October from 5:00
to 9:00 p.m.
3. To arrange for diagnostic reading by a radiologist.
4. To schedule mammograms. Interested persons should call the
North Colorado Medical Center Scheduling Center at 350-6070
and the caller should identify herself as part of the Weld
County Mammography Screen".
5. To provide a mammography and osteoporosis education session.
6. To send mammogram reports to the participant's private
physician.
7. To provide written confirmation of the following no less
than six weeks before the program begins:
a. Date, time, location, and cost of the educational
session.
b. Specific dates for the mammography screening.
8. To bill Weld County $55 for each mammogram less the $15
employee payment.
9. To coordinate with Greeley X-Ray the billing of a special
clinic fee of $20 for physician reading of each mammogram.
Responsibilities of the Weld County Personnel Office
1. To promote the program to Weld County Employees.
2. To provide a payment not to exceed $55/contact hour for
the educational program.

3. To promote the program at a special corporate rate of $55
for the mamogram, provided that employees and spouses
schedule during the dates and times set aside for
Weld County.
Responsibilities of Participants in the Program
1. To schedule their appointment at the education session
or through the North Colorado Medical Center Scheduling
Center 350-6070.
2. To provide a payment of $15 at the time of service.
3. To give North Colorado Medical Center their physician's
name and address so that reports can be sent to him/her.
4. To call their private physician one week after the
mammogram to receive the results.
If you are in agreement with these terms, please sign on the line
below and return one copy to the Education Department of North Colorado
Medical Center by September 2, 1987.
