Subject. Employment Services Full Service Agreement Between Employment
Services of Weld County and Centennial Health Care Facility
Enclosed for Board approval is a non-financial full service agreement
between Employment Services and Centennial Health Care Facility located at
1637 29th Avenue, Greeley, Colorado.
This agreement outlines the agreed upon duties of both the employer,
Centennial Health Care and Employment Services of Weld County concerning
the various employment and training initiatives available. It will remain
in effect until cancelled in writing by either party.

EMPLOYMENT SERVICES FULL SERVICE AGREEMENT
THIS AGREEMENT, made and entered into this 14th day of September ,
19 87 , by and between Centennial Health Care Facilit ,
whose address is 163 t venue, ree ey, o ora o ,
hereinafter referred to as "Employer," and the Board of County Commissioners of
Weld County, Colorado, by and on behalf of Employment Services of Weld County,
hereinafter referred to as "Employment Services."
WITNESSETH:
WHEREAS, Employment Services is agent for the Colorado Job Service, the Job
Training Partnership Act, and other employment and training initiatives, and
WHEREAS, Employment Services provides certain job search services to
employers pursuant to the above stated authorities, and
WHEREAS, Employer desires to utilize said job search services.
NOW THEREFORE, in consideration of the promises and mutual covenants
contained herein, the parties hereto agree as follows:
A. DUTIES OF EMPLOYER:
1. To provide Employer' s screening and selection criteria and
job descriptions for those jobs covered by this Agreement.
2. To list all openings for specified jobs with the Employment
Services as the job openings occur.
3. To provide the Employment Services with blank Employer's
application forms, if appropriate.
4. To direct all walk-in applicants (gate applicants) seeking
employment in specified jobs at Employer's company to apply at
Employment Services.
5. To notify Employment Services as soon as possible when decisions
about applicants referred have been reached.
6. To notify the Employment Services of any change about applicants
referred, job content, or selection criteria.
7. To maintain final responsibility for hiring decisions and
compliance with Equal Employment Opportunity (EEO) laws and
associated regulations.

8. To submit to Employment Services a written request for Targeted
Jobs Tax Credit (TJTC) benefit ON OR BEFORE DATE APPLICANT STARTS
WORK. This may be done by:
a. post-marked envelope (metered mail not acceptable without U.S.
Post Office cancellation mark)
b. hand-delivered envelope which has been date stamped in at the
Employment Services of Weld County.
B. DUTIES OF EMPLOYMENT SERVICES;
1. To designate an Employment Services staff person (or persons) as
primary contact with Employer.
2. To provide orientation to Employment Services staff who will be
dealing with Employer to avoid any misunderstanding relative to the
Full Service Agreement.
3. To explain to Employer the Employment Services procedures regarding
referral and placement regulations.
4. To identify special programs, such as On-the-Job training (OJT) ,
that by hiring eligible applicants, monetary incentives are
available to Employer.
5. To prescreen each applicant against the requirements of the job
order and have those individuals who pass the screening complete a
company application form when appropriate.
6. To expose the job order information to qualified and interested
Employment Services' applicants. Veterans will be given the
required priority of referral .
7. To provide testing and counseling services, if applicable.
8. To encourage qualified veterans, minority group members, women, and
handicapped persons to apply for Employer' s openings as a means of
assisting Employer with Affirmative Action goals. However,
qualified applicants, regardless of applicant group, will not be
denied referral .
9. To provide Employer with qualified applicant screening in order to
reach a goal of (as requested) or less qualified referrals for
each job available.
10. To provide pre-screening and vouchering, as appropriate, for
Targeted Jobs Tax Credit program eligibility for applicants prior to
referral .

C. INDEMNIFICATION:
Employer agrees to indemnify, hold harmless, and defend the Board of
County Commissioners of Weld County, Employment Services, and its
officers and employees from any and all loss and/or liability caused by
the actions or omissions of Employer, its employees, volunteers,
and/or agents while performing the duties as described in this
agreement. OO
