RE: APPROVE AMENDMENT NO. III TO VISION CARE ASSISTANCE PROGRAM
OF THE WELD COUNTY EMPLOYEE BENEFIT TRUST FUND AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment No. III
to the Vision Care Assistance Program of the Weld County Employee
Benefit Trust Fund, and
WHEREAS, Amendment No. III provides that eligible employees
and their dependents who participate in Weld County' s Peak HMO may
also participate in the Vision Care Assistance Program, and
WHEREAS, after study and review, the Board deems it advisable
to approve Amendment No. III to said Plan, a copy being attached
hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. III to
the Vision Care Assistance Program of the Weld County Employee
Benefit Trust Fund be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Amendment No. III .
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 6th day of
April, A.D. , 1987, nunc pro tuns January 1 , 1987.

IT IS AGREED BY WELD COUNTY that the provisions contained in the above document
and Amendment No. III thereto, are acceptable and will be the basis for the
administration of the WELD COUNTY EMPLOYEE BENEFIT TRUST FUND described herein.
