Head Start and Dr. Bill Bassore
Enclosed for Board approval is a Purchase of Services Agreement between
Weld County Division of Human Resources' Head Start Program and
Dr. Bill Bassore.
Dr. Basscre will develop a plan for obtaining construction materials and
will supervise actual construction of a preschool playground for the
Head Start Program.
The monies used to fund this project were awarded by the Department of
Health and Human Services. Head Start will reimburse Dr. Bassore at a
rate of $12.50 per hour for an amount not to exceed $4.000.00.
The term of this Agreement shall be from December 1, 1988 through
May 31 , 1989.

PURCHASE OF SERVICES
THIS AGREEMENT, made and entered into this 6th day of
February , 1989, by and between the Weld County Division of
Human Resources ' Head Start Program, Weld County Government, hereinafter
referred to as "Head Start," and Dr. Bill Bassore, hereinafter referred
to as "Contractor."
WITNESSETH
WHEREAS, Head Start has been awarded monies to construct a
playground for the Head Start participants, and
WHEREAS, the Contractor, as a preschool playground specialist,
desires to provide supervision of the playground construction project .
NOW, THEREFORE, in consideration of the mutual covenants contained
herein, the parties hereto agree as follows:
1. The Contractor will attend a minimum of three community
meetings, set up by Head Start staff, to give presentations concerning
the playground project.
2. The Contractor will develop: an organizational chart,
identifying all the personnel involved in the project; and a time chart,
showing the stages of the project.
3. The Contractor will design the various centers or components of
the playground and ensure that each meets the specific developmental and
educational needs of children according to their age.
4. The Contractor will develop a plan for obtaining construction
materials for the playground site, including purchasing of materials and
transporting to the playground site.
5. The Contractor will supervise the actual construction of the
project.
6. The Contractor will develop a training plan for Head Start
staff, parents, and community people on the effective use of the
playground.
7. The Contractor will develop, with input from Head Start staff
and community volunteers, a regular maintenance plan for the playground.
8. The Contractor will submit a monthly statement to Head Start
for performance under this Agreement. The statement will clearly
itemize the hours served by the Contractor.
9. Head Start agrees to reimburse the Contractor at the rate of
$12.50 per hour for an amount not to exceed $4,000.00 ($12.50 per hour x
320 hours = $4,000.00)
10. The term of this Agreement shall he from December 1, 1988
through May 31 , 1989.

11. The Contractor agrees that he is an independent contractor and
neither he nor his officers and employees become employees of Weld
County, and therefore are not entitled to any employee benefits as Weld
County employees, as the result of the execution of this Agreement.
12. This Agreement may be amended only upon written agreement of
both parties.
13. This Agreement may be terminated upon thirty (30) days notice
by either party.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement
as of the date first above written.
