Speckman, Executive Director, Human Resources" subject: Purchase of Service Agreement With School District RE-12 Enclosed for Board approval is the Purchase of Services Agreement between the Weld County Division of Human Resources' Employment Opportunities Delivery System (EODS) and Weld County School District RE-12. A special $2,235 Job Training Partnership Act discretionary grant has been approved by the Governor's Job Training Office. The grant enables EODS to act as the adminitrative entity and supply a specific job training program to the youth enrolled in RE-12 schools. The grant is effective until June 30, 1986. If you have any questions regarding the enclosed agreement, please do not hesitate to contact me. 850904 oDcs- (0. /pigs / fl PURCHASE OF SERVICES AGREEMENT THIS AGREEMENT, made and entered into this day of April , 1985, by and between the Weld County Division of Human esources' Employment Opportunities Delivery System , hereinafter referred to as "EODS" , and Weld County School District 12, hereinafter referred to as "Project. Site" , WITNESSETH: WHEREAS, EODS delivers employment and training programs and services to youth throughout Weld County; and WHEREAS, EODS is the Service Delivery Area (SDA) for all Weld County Job Training Partnership Act (JTPA) funds; and WHEREAS, the Project Site provides programs and services to youth within its school district; and WHEREAS, EODS is the grant recipient and administering agency; and WHEREAS, the Project Site submitted an application for the discretionary monies in a single application process to EODS; and WHEREAS, The Project Site was awarded through EODS, discretionary Job Training Partnership Act monies to run a special youth employment program within the school district boundaries; and WHEREAS, the Project Site understands the special youth program will be run through a purchase of services agreement with EODS; NOW THEREFORE, in consideration of the premises , the parties hereto covenant and agree to as follows: 1. The purpose of this Agreement is to establish a purchase of services arrangement between EODS and the Project Site whereby the goal identified in the Project Site's application can be carried out. The goal is: "To expose rural students to agencies, employers, and career choices in the city" . The objectives to be accomplished are: A. The participating youth will have a first hand introduction to employers with small private businesses , personnel directors of large businesses , and to public personnel directors. B. The participating youth will become aware of the application requirements for various size employers, gain confidence in meeting employers , gain confidence in applying and interviewing for positions available in their businesses and understand the correct methods of grooming, dress , and job maintenance techniques. C. The participating youth will be aware of the agencies that provide job counseling, testing, evaluation, and job opening listings. 2. The time frame for the project will be from April 10, 1985 through June 30, 1986. 3. EODS will honor expenditures for the project when proper receipts are presented and provided the expenditures fall into the categories identified below: A. Personnel Costs $ 200.00 (for the Project Site's substitute teaching staff) B. Travel Expenses 585.00 (for the Project Site) C. Equipment 200.00 (Equipment and materials will be purchased by EODS) D. Client Training 1,250.00 6 (includes but is not limited to the following) : o Reimbursement for meals for participating youth and supervisory staff ($300.00) o Reimbursement to EODS for the Training Facility ($700.00) o Additional resource materials, instructors , etc. ($250.00) E. TOTAL 2,235.00 Any expenditures above 52,235.00 will be the responsibility of the Project Site. Any monies not expended after the final workshop held in February, 1986, will revert to EODS and may be used to provide additional client training as seen appropriate by EODS. 4. EODS and the Project Site will co-sponsor three Job Search Workshops for the Project Site' s students enrolled in grades nine through twelve. The Project Site will enroll a minimum of twenty youth in the program. A minimum of 75% of all youth participating in the program will be economically disadvantaged. p•1 EODS will complete the youth' s application during a one on one intake session that will be provided for each youth. The Project Site will ensure that the youth obtain parental signatures on applications and return the applications to EODS for final eligibility determination. EODS will certify' all participating youth 's eligibility. The first workshop will be held on April 10, 1985. The remaining two workshops will be held (one each) in October, 1985, and February, 1986. The exact date of the workshops in October and February will be agreed upon by EODS and the Project Site six weeks prior to each workshop. • 5. EODS and the Project Site agree to the design of the project which includes but is not limited to: A. To expose rural students to agencies , employers and career choices in the city. B. EODS will provide the training site in Greeley, Colorado. C. EODS will provide an individualized client application session with each student. The application session will be conducted through the normal EODS intake process so that the student will be able to experience the "real life" situation of an application for employment. D. EODS will provide GATB Testing to each student. E. EODS will provide group Job Search Classes for the students and also conduct mock employer interviews. F. EODS will provide all equipment necessary for the project. G. The Project Site will provide all transportation for the students to and from the training site. The starting time for each training session will be 10:00 a.m. H. The Project Site will supply all personnel needed to provide the necessary supervision for the students while they are on their way to and from the training site and while at the training site. 6. The Project Site will conduct evaluation reviews on each youth participating in the project and will provide a quarterly report to EODS. The format of the report will be constructed in mutual cooperation between EODS and the Project Site prior to May 31, 1985. 7. EODS and the Project Site consent that this agreement may be amended at any time with the mutual written consent of both parties. 8. EODS and the Project Site consent that this agreement may be terminated upon 30 days written notice by either party. Furthermore, this agreement and all contracts associated with this agreement will be terminated immediately ifthe discretionary funding provided by the Job Training Partnership Act for this project is withdrawn by the Governor' s Job Training Office or the U.S. Department of Labor. IN WITNESS WHEREOF, the parties hereunto have caused this agreement to be duly executed as of the day and year first hereinabove set forth.
