Subject Head Start Dental Agreements
Enclosed for Board approval are five contracts with local dentists to
perform dental care on Head Start children.
Head Start has an agreement with Sunrise Community Health Center to
perform the dental care for the Head Start children. The health center
was not able to schedule dental appointments to handle all of the Head
Start children by the end of the school year.
Because Head Start is bound by federal rules and regulations to have all
dental care performed by the end of the school year, additional help is
needed to complete the appointments.
The dentists listed below have agreed to provide the needed dental
services for $40.00 per child:
1. Carol Potter, DDS
2. Bill Bender, DDS
3. Steven Bagley, DDS
4. Robert Benke, DDS
5. H. Kent Everett, DDS
Arrangements are currently underway with the health center to avoid this
problem in future Head Start school years.

MEMORANDUM OF AGREEMENT
THIS AGREEMENT, made and entered into this 20th day of May, 1985, by and
between the Weld County Division of Human Resources' Head Start Program,
hereinafter referred to as "Head Start," and Kent H. Everett, D.D.S.
hereinafter referred to as the "Dentist. "
In consideration of the mutual promises and covenants contained herein,
Head Start and the Dentist agree as follows:
1 . Head Start agrees to transport Head. Start children to the Dentist to
meet scheduled appointments at the Dentist's Office.
2. The Dentist shall provide prophylaxis therapy and application of topical
flouride, for each Head Start child. The Dentist agrees to provide all restoration
and/or extractions as he deems necessary on each referred child.
3. Head Start agrees to reimburse the Dentist $40.00 per child for these
services.
4. Head Start shall not be charged for those children covered by Public
Assistance. Medicaid numbers shall be provided at the time of the appointment
and billing shall be sent to the appropriate agency by the Dentist.
5. For services not reimbursed by Head Start, the amount will be designated
as In-Kind Contribution to the Head Start Program.
6. The term of the Agreement is April 1, 1985 through June 15, 1985.
7. The Dentist and Head Start mutually agree to accept that this Agreement
shall be terminated upon June 15, 1985.
