RE: APPROVE AMENDMENT NO. I TO VISION CARE PLAN OF THE WELD
COUNTY EMPLOYEE BENEFIT FUND AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS , the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS , the Board has been presented with Amendment No. I to
the Vision Care Plan of the Weld County Employee Benefit Fund ,
and
WHEREAS, Amendment No. I replaces Item #16 on Page 8 and
provides that Photogray Lens prescription glasses will not be
considered sunglasses, and
WHEREAS, after study and review, the Board deems it advisable
to approve Amendment No. I to said Plan, a copy of which is
attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that Amendment No. I to
the Vision Care Plan of the Weld County Employee Benefit Fund be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is , authorized to sign said Amendment No. I.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 3rd day of

AMENDMENT NO. I
to the Health Plan Document of
THE WELD COUNTY EMPLOYEE BENEFIT FUND
The Health Plan Document of Weld County Employee Benefit Fund is hereby amended,
effective January 1, 1986, as follows :
Page 8 - The following paragraph(s) is hereby deleted in its entirety:
Item #16
Page 8 - The following paragraph(s) is hereby added in its entirety:
Item #16
Sunglasses, plain or prescription. Tinted glasses with a
tint other than No. 1 or No. 2 will be considered sunglasses
for this purpose. Photogray Lens prescription glasses will
not be considered sunglasses for this limitation; or
IT IS AGREED BY WELD COUNTY that the provisions contained in the Plan Document
and Amendment No. I thereto are acceptable and will be the basis for the admi-
nistration of said Employer' s Employee Benefit Program described herein.
