The Memorandum of Understanding outlines the process for referrals to the Area Agency Case Management Program and the obligations of the Area Agency in designing and implementing individual care plans for clients. The agreement shall stay in effect for the time period of March 1 , 1989 through December 31 , 1989, unless otherwise amended. Please telephone Linda Piper at 353-3816 if you have any questions or concerns. 89^2C3 MEMORANDUM OF UNDERSTANDING THIS MEMORANDUM OF UNDERSTANDING, made this 27th day of February 1989, by and between the Weld County Area Agency on Aging Case Management Program, hereinafter designated as "Carelink," and North Colorado Medical Center, hereinafter designated as "Agency." WITNESSETH: WHEREAS, "Carelink" and the "Agency" desire to cooperate to achieve maximum efficiency and effectiveness among all agencies serving the high-risk elderly population of Weld County, and WHEREAS, "Carelink" provides ongoing comprehensive case management services to high-risk elders, and WHEREAS. the "Agency" provides programs, activities, and services designed for maximizing the independent functioning of Weld County elderly, and WHEREAS, "Carelink" and the "Agency" desire to work together for the express purpose of increasing the availability of case management services to Weld County elderly and their families. NOW, THEREFORE, in consideration of these premises, the parties hereto covenant and agree as follows: 1. Carelink agrees to: a. Provide a single entry case management service to include both social and health service linkages to high-risk elderly of Weld County. b. Assess and reassess the health and social needs of qualified elders over the age of 60 years. c. Develop an objective care plan to meet the needs of the target population and make adjustments to the plan, as necessary. d. Provide ongoing case management services with formalized reevaluation procedures. e. Provide technical assistance and information to the "Agency," as requested and available, regarding the needs of the Weld County elderly. f. Provide the "Agency" with a monthly list of "Carelink" clients in order to increase coordination of services among programs. 2. The Agency agrees to: a. Screen clients and refer appropriate clients to Carelink, with clients ' approval . b. Accept and serve, when possible, high-risk elders in accordance with arrangements provided by "Carelink's" brokering function. 89023 c. Provide "Carelink" with ongoing feedback on participants , in relation to services provided. d. Participate in planning meetings with "Carelink" staff to set goals and priorities for services in Weld County for the target population. e. Notify "Carelink" if unable, for any reason, to perform the services agreed to in the care plan. f. Assure the quality of service delivered to "Carelink" participants, as specified in the care plan. g. Protect client confidentiality and not identify "Carelink" participants in any reports without prior consent of the client. 3. "Carelink" and the Board of Directors of the "Agency" shall have exclusive control of the management, assets, personnel , and affairs of their respective institutions. 4. This Memorandum of Understanding may be terminated upon thirty (30) days written notice by either party. 5. This Memorandum of Understanding covers the time period from March 1 , 1989 to December 31 , 1989.
