You are directed to make the following changes in the work. All other terms
and conditions of the contract not expressly modified hereby shall remain in
full force and effect.

Subject: cl i psh°et acrecsory
In order to facilitate using the Supplemental Foods forklift for both
slipsheet and palleted items, a quick disconnect kit and couplers are
needed to detach slipsheet attachment and connect pallet forks with
minimal time and effort.
The slipsheet plattens made for this type of forklift cannot be used
for both activities with grocery size pallets.
Without the disconnect kit it takes 45-90 minutes to change attachments
on the lift. With the kit it's just a matter of a flip on--flip off! !
The latter came to our attention when Jack Berenberg came to Greeley to
train Ray Lopez on the new slipsheet attachment.
It is imperative that we get this installed as soon as possible. Mr.
Berenberg has the part available and is willing to install it asap.
Attached is the Information Jack gave me to order the parts. Also
included is the original bid for the slipsheet.
