Subject: Purchase of Services Agreement between Weld County Head Start
and Anna Trujillo
Enclosed for Board approval is a Purchase of Services Agreement by and between
Weld County Division of Human Resources' Head Start Program and Anna Trujillo.
Ms. Trujillo will be providing training to the teaching staff so that they may
obtain Child Development Associate (CDA) certification. The contractor will be
reimbursed at a rate of $11.00 per hour for 24 hours of service for a maximum
reimbursement of $4,224.00.
The term of this Agreement is for the period March 6, 1989 through June 30, 1989.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this 12th day of April ,
1989, by and between Weld County Division of Human Resources ' Head Start
Program, hereinafter referred to as "Head Start," and Anna Trujillo,
hereinafter referred to as "Contractor."
WITNESSETH
WHEREAS, Head Start is requiring teaching staff to obtain their
Child Development Associate (CDA) certification, and
WHEREAS, the Contractor is desirous of providing CDA training.
NOW, THEREFORE, in consideration of the premises and mutual
covenants contained herein, the parties hereto covenant and agree as
follows:
1. TERM
This Agreement shall be in effect for the period March 6, 1989
through June 30, 1989.
2. FUNDING
The Contractor will be reimbursed at a rate of $11.00 per hour for
24 hours of service per week for a maximum of $4,224.00 (24
hours/week x 16 weeks x $11.00 per hour = $4,224.00) .
3. HEAD START RESPONSIBILITY
Provide reimbursement for services rendered by the Contractor, as
billed monthly. Reimbursement will be made within fourteen (14)
days of submission of invoice.
4. CONTRACTOR RESPONSIBILITIES
a. Assist CDA candidates in portfolio development
b. Assist candidates in preparation for Local Assessment Team
(L.A.T. ) review
c. Conduct a mock L.A.T. review for candidates
d. Complete observations and give feedback to clients. Fill out
advisor report forms.
e. Assist two (2) parent representatives in completing
observations and report forms.
g. Submit monthly invoices to Weld County Division of Human

5. MODIFICATION OF AGREEMENT
All modifications to the Agreement shall be in writing and signed
by the parties hereto.
6. ASSIGNMENTS
This Agreement shall be binding upon the parties hereto, their
successors, heirs, legal representatives, and assigns. The
Contractor may not assign any of her rights or obligations
hereunder without the prior written consent of the Weld County
Commissioners.
7. ASSURANCES
a. The Contractor agrees that she is an independent contractor
and neither she nor her officers and employees become
employees of Weld County, and therefore are not entitled to
any employee benefits as Weld County employees, as the result
of the execution of this Agreement.
b. Weld County, the Board of County Commissioners of Weld County,
its officers and employees, shall not be held liable for
injuries or damages caused by any negligent acts or omissions
of the Contractor, or her employees , volunteers , or agents
while performing Contractor's duties as described in this
Agreement. The Contractor shall indemnify, defend, and hold
harmless Weld County, the Board of County Commissioners of
Weld County, its officers and employees for any loss
occasioned as a result of the performance of this Agreement by
Contractor, her employees, volunteers, and agents.
c. No officer, member, or employee of Weld County, and no member
of their governing bodies, shall have any pecuniary interest,
direct or indirect, in the approved Agreement or the proceeds
thereof.
d. The Contractor understands that monitoring and evaluation of
the performance of the Agreement shall be conducted by Human
Resources and the results provided to the Board of County
Commissioners of Weld County.
e. The Contractor assures that she will comply with Title VI of
the Civil Rights Act of 1986, and that no person shall , on the
grounds of race, creed, color, sex, or national origin, be
excluded from participation in or be otherwise subjected to
discrimination under this Agreement.
9. ENTIRE AGREEMENT
This Agreement constitutes the entire understanding between the
parties with respect to the subject matter hereof, and may not be

changed or modified except as stated in Paragraph 5 herein.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement
as of the day and year first above written.
