RE: APPROVAL OF CONTRACT BETWEEN THE WELD COUNTY DEPARTMENT OF
SOCIAL SERVICES AND THE WELD COUNTY ASSOCIATION OF OPTOME-
TRISTS AND OPHTHALMOLOGISTS
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, a vision care contract for 1980 between the Weld
County Department of Social Services and the Weld County Associa-
tion of Optometrists and Ophthalmologists has been presented to
the Board of County Commissioners of Weld County, Colorado, and
WHEREAS, the Board of County Commissioners has studied said
contract and deems it advisable and in the best interests of
Weld County to approve the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the vision care con-
tract for 1980 between the Weld County Department of Social
Services and the Weld County Association of Optometrists and
Ophthalmologists be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 23rd day of
January, A.D. , 1980.

VISION CARE CONTRACT 1980
This contract executed at Greeley, Colorado this 3/9# day of December, 1979, by and
between the Board of County Commissioners of Weld County, Colorado, hereinafter called
the County, and the Weld County Association of Optometrists and Ophthalmologists,
hereinafter called the Association.
WITNESSETH:
That in consideration of the mutual promises and obligations and other considerations
herein expressed, it is mutually agreed until December 31, 1980 that:
1. All eyeglass services shall be provided by prior written authorization by the
Weld County Department of Social Services .
Participating optomesxrists and ophthalmologists, asyall as social services staff
must cooperatively monitor use of this program in order to utilize the limited
funds effectively and efficiently.
2. Children of General Assistance recipients and eligible adults will be provided
glasses through this contract within funding limitations.
MEDICAID provides eyeglasses for children (up to the age of 21) of recipients of
Aid to Families with Dependent Children (AFDC) and for children in Foster Care.
3. The fee for examination is established at $22.00: The County to pay $18.00; the
patient to pay $4.00. If tonometry is essential, and is performed, the examining
doctor may bill an additional $4.00 for this service.
4. The cost of lenses must be determined by a standard lens service fee of $7.00 per
pair for single vision lenses; $10.00 per pair for bifocal lenses; and $13.00 per
pair for trifocal lenses, plus laboratory prescription prices for lenses from the
current Bausch and Lomb price list. Deluxe/Photogray lenses are excluded; if
chosen, the patient/recipient must pay any additional costs.
5. The cost of frames is determined by a standard frame service allowance of $10.00
plus actual laboratory cost of the frame (to $12.00 maximum) from the manufacturer's
current price list. If a lower priced frame is used, the county shall be billed
the lower cost. If a higher priced frame is used, the patient/recipient must pay
costs exceeding the $12.00 maximum allowance.
6. Temples and minor frame repairs not exceeding $5.00 in cost are to be billed at
customary charges. Lens and frame replacements are to be billed as stated in para-
graphs (4) and (5).
7. The patient/recipient must pay $4.00 for the examination plus all other costs ex-
ceeding the maximum allowances - before receipt of the lenses and frames.
8. When submitting vouchers for payment, authorization forms and itemized statements
must accompany the vouchers . Along with the fees for services and laboratory
costs, the statement must show the patient/recipient's name, prescription, type
of lenses provided (single vision, bifocal, or trifocal), and amounts paid by the
patient. Failure to comply with these provisions will result in disapproval by
the adjudicating committee and non-payment of the billing.
9. A committee consisting of representatives of the Optometrist and Ophthalmological
professions will recommend policies and initiate procedures of the program and will

10. Only members in good standing in their respective and accredited society and/or
association will receive appointments as a participating member of the Social
Services Vision Care program.
11. Total amount paid by the County shall not exceed $16,000 for the year 1980.
When this budgeted amount is reached, no provision is made for further services.
IN WITNESS WHEREOF, said parties have subscribed and executed these presents , the
County in its proper name and by its proper authorized officers, attested by its
clerk and Sealed, and said Association by its proper name and proper officers there-
unto duly authorized by it, the date first above mentioned .
