RE: APPROVAL OF CONTRACT BETWEEN THE WELD COUNTY DEPARTMENT OF
SOCIAL SERVICES AND DR. ROBERT D. HARTLEY.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, a contract for pediatric medical consultation ser-
vices between the Weld County Department of Social Services and
Dr. Robert D. Hartley has been presented to the Board of County
Commissioners, and
WHEREAS, the term of said contract shall be from January 1,
1981 through December 31, 1981, and
WHEREAS, the rate of payment shall be $50. 00 per hour and
the total amount of the contract shall not exceed $1 ,650 . 00 per
year, and
WHEREAS, the Board of County Commissioners deems it advis-
able and in the best interests of Weld County to approve said
contract.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the contract between
the Weld County Department of Social Services and Dr. Robert D.
Hartley for pediatric medical consultation services be, and hereby
is, approved.
The above and foregoing Resolution was , on motion duly made
and seconded, adopted by the following vote on the 3rd day of
December, A.D. , 1980.

2. This contract shall be effective from Jemmy 1, 1981 until
Decslber 31, 1981 . This agreement may be terminated in
advance of the expiration date specified upon 30 days prior written

this contract all accounts and payments will be processed according to
financial arrangements set forth herein for services rendered to date of

by the Contractor to the Weld County Child Abuse Team in partial fulfillment
of the County's statutory duty for coordination of the Team. Payment will
be made to the Contractor at the rate of S50 00 per hoer
(unit of service)
The total amount of this contract may not exceed $ 1,850;00 per year
4. County agrees:
a. To pay Contractor after satisfactory completion of one or more con-
sultations and receipt of the billing form provided by County Voucher
procedure.
b. To assure that the service described herein has been provided
prior to payment to Contractor.
5. Contractor agrees:
a. To provide the service 1oscribed herein at the location shown as the

b. Not to assign any provision of this contract to a subcontractor.

c. Not to charge clients any fees related to services provided under
this contract.
d. To hold the necessary license(s) which permits the performance of the
• service to be purchased herein.
e. To abide by provision of the Civil Rights Act of 1964 which
prohibits discrimination against any employee or applicant for
employment or discrimination against any client because of race,
• religion, color or national origin.
f. To comply with requirements for safeguarding client information in
accordance with rules of the State Department of Social Services.
g. Maintain iscal books, records, docanents relevent to this contract
for a period of five years or until a federal audit is completed,
whichever occurs later, after the final payment under this contract.
h. To provide access by county, state and federal officials to records
or other information pertaining to the contractor's provision of
service under this contract.
i. To provide the service described herein at cost not greater than that
charged to other persons in the sane community.
6. All terms and conditions are included in the contract; all items
incorporated by reference are physically attached.
This contract contains all the terns and conditions agreed upon by the
parties and includes Attachment Number(s) (1) . All items
incorporated by reference are attached. No other understandings, oral
or otherwise, regarding the subject matter of this agreement, shall be
deemed to exist or to bind any other parties hereto.

The specifics of this contract includes attendance at two (2) Child Protection
•
Tram meetings per month, providing medical and/or psychiatric consultation on
three to four cases per meeting.
The contract also includes consultation on crisis situations where a session is
needed to meet the special needs of an abused or neglected child.
