RE: APPROVAL OF SERVICE PLAN OF TRI AREA AMBULANCE DISTRICT
WHEREAS , the Board of County Commissioners of Weld County ,
Colorado , pursuant to Colorado statute and the Weld County Home
Rule Charter , is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS , proponents of the Tri Area Ambulance District ` filed
a Service Plan with the Board of County Commissioners of We= c'
County , Colorado, and
WHEREAS ;, the Board of County Commissioners of Weld County ,
Colorado, did , on October 10 , 1984 , set a hearing date of Nov<_snbei
7 , 1984 , at 2 : 00 p.m. , to consider said Tri Area Ambulinc,E
District Service Plan, and
WHEREAS , said hearing was held, pursuant to a Notice dbteC
October 15 , 1984 , and the Board heard the testimony and statement:
of those present, and heard the recommendations of the Weld County
Planning Commission , and
WHEREAS , it was determined at said hearing that the Long.non'
Fire Protection District. had not received a Notice of Hearing and
the Board deemed it advisable to continue this hearing to Novenbe:
21 , 1984 , at 2 : 00 p.m. , and
WHEREAS , at the public hearing of November 21 , 1984 , the
Board of County Commissioners heard the testimony and statetant ;
of the Longmont Fire Protection District and other interested
persons , and being fully informed, found that said Service Plan
meets the criteria set forth in Section 32-1-203 , CRS , as amended
and said Service Plan should be approved.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that the Tri Arco
Ambulance District Service Plan be , and hereby is , approved as s.=I.

NOTICE IS HEREBY GIVEN , the Board of Weld County Commissioners
will conduct a public hearing to consider the request for the
formation of an ambulance district in southwest Weld County .
The hearing has been set for November 7 , 1934 , to be held in
the First Floor Hearing Boom, Weld County Centennial Center ,
915 10th Street , Greeley, Colorado, at 2 : 00 P .M.
The proposed TriArea Ambulance District will consist of the
Frederick Fire Protection District and portions of the Dacono
Area Fire Protection District, encompassing the municipalities
of Frederick, Firestone and Dacono and the unincorporated -,ni'2a
of Evanston.

THE DEPARTMENT OF PLANNING SERVICES STAFF RECOMMENDS THAT THIS REQUEST BE
APPROVED FOR THE FOLLOWING REASONS:
Section 32-1-203 et seq. CRS as amended sets forth eight (8) criteria for
review of special district service plans. The following criteria have been
satisfied:
1 . The proposal is in compliance with the master plan (County
Comprehensive Plan) adopted pursuant to Section 30-28—I.10 CRS

In the creation of a Special District, the Planning Commission has the

district in the Tri-Town area. The Planning Commission and the Board
will need to review the proposal as outlined in CRS 1973, Section

Planning Commission and the Board to determine if the proposal is
compatible with the Comprehensive Plan and if there is a need.

favorably upon this proposal; such a district will provide emergency medical services and
transportation of sick, disabled and injured persons to and from facilities with con )lets

The sponsoring Tri Area Volunteer Ambulance Service (TAVAS) is not tax >up>:erted
and can no longer meet the financial requirements of keeping pace with modern-day
training, medicine and equipment.

IN THE MATTER OF THE FORMATION )

DISTRICT ) SERVICE PLAN
WHEREAS, Pursuant to C.R.S. 32-1-101 the resident taxpaying electors of
the above proposed district intend to file with the District Court in and for the Counts of
Weld and State of Colorado, a Petition for Organization of the Tri Area Arnbular ce
District; and,
WHEREAS, C.R.S. 32-1-202 requires that all proposed special sery cc
districts file a Service Plan with the County Commissioners of the county in which ,he
territories of the proposed district are included;
NOW, THEREFORE, the sponsoring Tri Area Volunteer Ambulance Sery ce
(TAVAS), in compliance with said law, submits this Service Plan:
PROPOSED SERVICE AREA
The Tri Area Ambulance District is proposed to contain forty-three sections
of southwest Weld County, more particularly identified and illustrated by the -nap in3d
legal description attached as Exhibit A.
Boundaries of the district are calculated to be the same as those of the
Dacono Area Fire Protection District and the Frederick Fire Protection District, n id
thereby to encompass the municipalities of Frederick, Firestone and Daco.-to.
The population of the entire area mapped out is estimated to be 7,000, witl a
1983 assessed valuation of more than forty-nine million dollars.
JUSTIFICATION
At the present time the only ambulance service located within the requester
district area is that of TAVAS. Operations are at Frederick, the heart of the Tri fowni
of Frederick, Firestone and Dacono.
These communities have relied upon TAVAS for ambulance service since it
inception in 1971, and have supported that service with contributions, membenships, am-
with participation as volunteers.
Currently TAVAS has 17 volunteers, experienced and well-trained. Seine are
ten year veterans, most have at least four years service, and in experience anc. aciv•rncei
training the group boasts Emergency Medical Technicians, Emergency NI xdical
Technician Intermediates and two Paramedics.
Outside ambulance units are Paramedic 3 at Fort Lupton, and the Longmont
Professional Ambulance Service. Both come into the Tri Town region from time to time,
primarily on calls requiring an Advanced Life Support (ALS) capacity, since TAVAS is and
has been a Basic Life Support (BLS) unit.
The BLS ability alone has justified the thirteen year existence of TAVAS.
Experience reveals that 95% of all ambulance calls are of the BLS variety, and TAVAS
believes its quick response in many instances has kept life support requirements from
escalating. Paramedic 3 and Longmont Professional Ambulance are miles farther away
and necessarily several minutes longer (7-10 critical minutes) in response time.
TAVAS equipment and facilities have been continually expanded and
improved. Quarters are in a two story block structure built on land provided by the Town
of Frederick. There are kitchen and washroom facilities, a television room, meeting
room, and sleeping quarters for the 24 hour staff. In the garage are two $50,000
ambulances, complete with Basic Life Support equipment, and some Advanced Life
Support components, including cardiac monitor, and intravenous and airway equ,prre,it.
Despite the substantial gains TAVAS has made over the years, it is financially
unable to obtain the level of services required of an ALS unit. From a "state of the art"
perspective, the equipment is complete and staff well trained, but the area is no ionger
suited to manpower and financial dependence solely upon volunteer help nd
membership, low rates and charity.
TAVAS remains a Basic Life Support unit, and a very good one, but the times
call for more special equipment, with more training, costly medicines and, the o atom
line, more money.
The Tri Area has a significant number of elderly and fixed income residents,
statistically people most often requiring emergency medical assistance and least often
able to pay a great deal for it. Also, it is a high growth area because it is located just
off 1-25, and, also because it is located near I--25, traffic accidents with severe injuries
are commonplace.
Fire Districts historically and legally are the agencies respDnsible for
emergency aid, they indeed remain the primary source in this regard. Certainly' creation
of another taxing district would be foolhardy without support of affected fire districts.
Cooperation is the key, as is cooperation by intergovernmental agreement, not
duplication and proliferation of special districts.
In the case at hand, TAVAS has such support (Exhibit B) and cooperation
(Exhibit C).
FINANCIAL FEASIBILITY
For TAVAS and for the 'Fri Towns, the hope eternal has been that E. funding
source would surface. Now it has, in the form of 1982 Senate Bill 185, autho-izing
ambulance districts as taxing entities. Ambulance service in the Tri Towns can now
become what the communities and the times require - full Advanced Life Support
capability, coordinated with local fire districts, air transportation, adjacent ambulance
services, and other appropriate agencies.
As a special district and separate taxing unit, the Tri Area Ambulance
District can get the revenue necessary for the most advanced emergency medical
treatment and transportation - beginning in 1985, even though tax monies will not be
forthcoming until 1986. In the proposed 1985 budget (Exhibit D), expenses over revenues
can be met with a short term loan. The second year budget (Exhibit El calls for a tax
levy of three and one half mills, to pay off the loan, while still maintaining operations
full scale. Taxes thereafter will require reevaulation of all aspects of service — rates,
memberships, growth, facilities, manpower and equipment.
CONCLUSION
If reorganized as the Tri Area Ambulance District, TAVAS can immediately
upgrade its services, launch advanced training classes, and put full time paid Paramedics
and EMTs on staff. At the same time, volunteers will continue as a main support fo:•ce,
and benefit functions and membership drives will be ongoing to keep the costs down.

RESOLUTION EXHIBIT B
WHEREAS, the Frederick Area Fire Protection District, and the Dacono Area
Fire Protection District, have previously entered into mutual aid agreements with the Tri
Area Volunteer Ambulance Service, for the mutual benefit of each of the parties .n
providing rescue, extrication and medical attention to all persons within their respective
boundaries; and,
WHEREAS, the Tri Area Volunteer Ambulance Service, having Petitioned for
authority to operate as a special district in the areas of the said fire protection districts,
is desirous to continue with the mutual aid agreements aforementioned; and,
WHEREAS, it appears the creation of such special district and its taxing
authority will strengthen the capabilities for emergency medical treatmert aid
transportation in this area of Weld County;
NOW, THEREFORE, BE IT RESOLVED:
1. That the Tri-Area Volunteer Ambulance Service has the full support of
the Frederick Area Fire Protection District and the Dacono Area ''ire
Protection District, in its Petition for reorganization as the Tri-.^sea
Ambulance District, a special taxing authority.
2. That mutual aid agreements currently effective will be continded and/or
renewed with the said Tri-Area Ambulance District upon its creation.

AGREEMENT
THIS AGREEMENT, made and entered into this ".6 day
of ,,,,- , 1984, by and between the Dacono Area Fire Protection District, 100
Fore Street, Dacono, Colorado, and the 'Fri-Area Ambulance Service of Frederick,
Colorado and the Weld County Paramedic Ambulance Service of Greeley, W≥Id Co.,
Colorado;
WITNESSETH THAT:
Whereas the parties hereto maintain paid and or volunteer Emergency
Services Organizations, fire rescue, medical and ambulance, together with personnel and
equipment therefore needed, and,
Whereas, it is and will continue to be to the mutual benefit of each of the
parties to assist the other whenever necessary in providing personnel and equipment for
the purpose of rescue, extrication, and medical attention to all the persons within the
boundaries of the Dacono Area Fire Protection District, and in turn to receive such
assistance, each from the other;
NOW THEREFORE, in consideration of tine premises and the rnu?ua1
covenants, performances and agreements hereinafter set forth, it is mutually understood
and agreed by and between the parties of this agreement as follows;
(1) This agreement shall supercede and take the place of any and all
agreements now in effect between the Dacono Area Fire Protection
District and the 'Fri-Area Ambulance Service, and shall be in addition to
the agreements between the Dacono Area Fire Protection Districi and
the Weld County Paramedic Ambulance Service;
(2) As per Section 32-5-314, of Title 32, Article 5, Part 3 of the laws for
Colorado Fire Protection Districts, and the Powers therein ;ranted,
Specifically sections (d), (0, (h), and (i) of this section, the parties to this
agreement herein agree to the following conditions and standards of
operations within the boundaries of the jurisdiction of the Dacono Area
Fire Protection District;
(3) Each of the parties, upon receiving a call for medical service or
extrication or the need for each or the other to respond nto the
jurisdiction of the Dacono Area Fire Protection District, shall
immediately after giving said notice of the need to respond to their own
agency or organization, then give notice of the need to respond to the
other agencies or organizations to this agreement, in keeping Nit'r the
requirements of this agreement, and
(4) Each of the parties to this agreement shall to the best Df their
capability, keep the other informed of the demands, type and needs of
the call or response by a previously designated method of
communications.
(5) Dacono Area Fire Protection District, by virtue of its personnel,
training, and equipment shall furnish first responder Emergency Medical
Services and stabilization of the patient, and all extrication, fire
fighting, fire protection and rescue as needed, and the Tri-Area
Ambulance Service shall upon its arrival take charge of the Medical
treatm nt an;i the Trens^nrte!ion of the 1-atient or patients,

(6) Tri-Area Ambulance Service will respond to all calls within the
established district, namely the Dacono Area Fire Protection District,
and upon arrival or notification prior to arrival, of the need of further
assistance or expertise, and or the need of Air Transport Services:, shall
through mutual and co-operative agreements, make necessary calls to
receive the additional needed equipment or personnel, end shall keep
each of the responding agencies completely advised of any and ad such
calls for additional responding units,
(7) In order that this agreement shall remain in full force and effect, it is to
be understood that the Weld County Paramedic Ambulance Service will
be the designated back up service agency for assisting Tri-Ar^.a
Ambulance Service and the Dacono Area Fire protection District on all
calls where response time of not more than seven minutes may be
reasonably anticipated. However, when circumstances prevent su,•h
response time, Weld County Paramedic Ambulance Service shall decline
back up and notify Greeley Communications to dispatch Saint Anthony's
Flight For Life or Colorado Medical Center Helicopter Support Sera he
(Air Life), as may be required and declared to be most available and
fastest in the area needed, and,
(8) All calls for either of the Helicopter Air Transport units, as may be
needed, will be filtered through the Dacono Area Fire Protection District
Officer on scene, and all air support units will be requested through the
normal communications channels utilized by the parties to the
agreement, Weld County Communications Center, and all ineomiag
Helicopter units will be palced on the Weld County Fire A Channel, or in
emergency cases, on the FERN Channel, 154.280, but only in the event
that Weld County Fire A for any reason is incapable of handling he
traffic or communications, and the Daconc Area Fire Protection Distr ct
Officer or Unit on scene will be delegated as landing zone co-ordinator
for the scene, in keeping with the ::tandards of operations and
communications now in effect in the Weld County Comrnuniaations
Center, and,
(9) It shall also be understood that throughout the duration of thus
agreement, the Tri-Area Ambulance Service will be the primary
transport agency for the Dacono Area Fire Protection District, and Tri-
Area Ambulance Service will be required to have on its staff during the
daytime hours of operations, one EMT or qualified diver, and one
Certified Paramedic, seven days a week, working out of a fully equipped
Advanced Life Support Ambulance, beginning no later than June 30,
1984, and beginning no later than January 30, 1985, they shall have no
. less than one EMT or qualified driver and one Certified pare in cdi on
said ambulance at all times, 24 hours per day, and,
(10) In the event of sickness or other emergency for short term duration; not
to exceed two working days, the above requirements may be exempted,
but only if the Dacono Area Fire Protection District is notified
immediately of the absence and if the Weld County Paramedic
Ambulance Serivee is duly notfied as the need to be available to cover
all calls as needu 1, and

(11) Further, it shall be the requirement, to keep this agreement in fell force
and effect, that the Tri-Area Ambulance will, beginning in June, 7 b84,
have in its employ at all times a Certified Paramedic Attendant, and in
January 1985, shall improve the service to full time Paramedic service as
above stated, and,
(12) Failure to maintain the above agreed minimums, shall immediately upon
notification, Revoke and Declare this and all other agreements, between
the Tri-Area Ambulance Service and the Dacono Area Fire Protection
District as to be finally terminated and void, and by this act.on will
automatically designate the Weld County Paramedic Ambulance Service
as the sole and only responding ambulance service serving the agreement
with the Dacono Area Fire Protection District, and
(13) The equipment and employees of each of the parties, while engaged in
performing any mutual aid service, activity, or undertaking ruder
provisions of this agreement, shall have and retain all rights, privileges
and immunity of and be deemed to be engaged in the service -md
employment of such party, notwithstanding that such mutual aid service
is being performed in or for the other party.
IN WITNESS WHEREOF, the parties have hereunto set their hands and seals

Per your request I have reviewed the financial capability criteria of tie
proposed Tri-Area Ambulance District. The limited information provided
makes it difficult to evaluate the financial information in depth. However,
based upon the information provided I would offer the following:
1. Revenue estimates based upon a call per day averaging $200 per call
with an 80% collection rate seems very optimistic. Based upon Weld
County Ambulance Service's experience in 1983 a more realistic estimate
would be 100-120 calls per year at $200 per call with an 80% collection
rate. This would result in revenues of $19,200/year versus $58 ,400, or
a reduction of $39,200 in anticipated revenues.
2. Projected salaries for staff are far below the market. In comparison
to the Weld County Ambulance Service's comparable salaries are es
follows:

3. The budget does not appear to provide for a replacement reserve for

5. Legal authority for a short term debt of $47,600 should be conCirmed
prior to incurring such debt in 1985.
6. It would appear that with more realistic revenue and expenditure
estimates that the 3.5 mill levy may not be adequate to support tie
district in 1936 and future years.

Dear Commissioners and Planning:
It would appear the map which accompanied Service Plan
for creation of the Tri Area Ambulance District is in error. Thus,
I an submitting herewith an amended map and legal description.
Let me point out that the slashed sections designate the
Frederick and Dacono Fire Protection Districts and constitute all
of the area proposed as an ambulance district. The shaded portion
is seeking exclusion from the Longmont Area Fire Protection District
and inclusion in the Dacono District, thus may one day also be brought
into the ambulance district, but not now.
There has also been mention that Exhibit B is incomplete,
i.e. , it is missing dates and a signature. I shall arrange for it
to be executed anew and bring it to the Planning Board meeting on
October 30.
Finally, I shall also bring to the meeting a letter from
the Fort Lupton bank, or some bank, respecting the short term loan
proposed in the Service Plan.

Re: Tri-Area Ambulance District
Dear Lee:
I believe I have gathered additional information you requested. These
people, of course, are in and out and it is very difficult to find one
person with all the information. I believe if we ever get to public hearing
several will attend and answer all questions.
Anyway, taking your inquiries in orders
(1) Operating expenses -- the big item is salary for two full-time
paramedic/EMT crews (48 hours on and 48 hours off) . Paramedics will be
paid $13,000.00 annually, and EMTs $4.00 hourly, which works out to a
total of $47,600.00. Add a full-time administrator at $10,000.00; 550.00
a month for each of 7 directors, plus insurance and benefits to total the
$70,230.03 set. wti in the proposed budget. The $12,000.00 for building
maintenance and utilities I am now told is high -- probably more like
$6,000.00, as is supplies by perhaps $5,000.00. Unit payments, maintenance
and insurance includes monthly payments of more than $600.00, on equipment,
a "Life-Pac 4" and a "Med-137".
(2) Costs of transfer -- the current volunteer service has an estimated
$250,000, in equipment. This would simply be given over to the District,
with almost no cost involved.
(3) Costs of creating district -- based upon the Recreation District
experience a couple of years ago it is believed $3,000.00 will take care
of costs such as legal fees, publication and election costs, which would
come under "Miscellaneous" in the proposed budget.
(4) Calls, memberships charges -- The petitioners anticipate an average
of one call per day at an average $200.00 per call or $58,400.00, at an
80% collection rate reflected by book keeping accounts.
This is three times what the volunteer service has been doing as
a Basic Life Support Unit, and may be overly optimistic. I trust
the Commissioners will get into this at the hearing. $50.00 member-
ships, though, should remain at the current number of about 100.
So we are talking $100,000.00 expense and $63,400 income the
first year. Property tax will make up the difference, plus provide
funds for new equipment down the road.
I really don't believe we can get any closer on these figures
until we get everyone together at a hearing.

On May 4, 1984, at about 9:45 PM, I was involved in a head-on
c i 1 i si on with another driver (who was charged with a DUI) on CO-S"
about one mile west of 1-25. I am not aware of who called which
emergency vehicles to the scene of the accident. I do know that one
nearby resident did g=all the Longmont Fire Protection District.
Following the accident, a nearby resident came to the scene of the
accident and drove my wife and son to his house. I remained at the
scene of the accident. At the resident' s house, the woman cleaned my
wife and son up and checked for signs of injuries.
At the scene of the accident, I recall counting 14 vehicles with
red or other colored blinking lights. No one other than the Colorado
State Patrol Officer identified himself (Officer Nash) to me or to my
wife. No one stated who called them or why they were there.
After my wife and son had been at the above resident's house for
a period of time, a woman, along with a man from the Brownsville Fire
Dept , entered the house. My wife assumed this woman was with the
Brownsville Fire Dept. also. The woman (in a uniform) started to
administer first aid to my wife and son. This help was not asked for.
Prior to our leaving, via this resident' s car, to go home and get
one of our other cars to go to Longmont United Hospital , one of the
ambulance personell asked me to sign a "release", which would free
them of "negligence". Again, the ambulance personell did not identify
themselves, nor ask me if I wished to sign, I was signing to "release
them of negligence".
Consequently, I have received a bill from your ambulance service.
I understand you are not a government sponsored agency but a private
service.
I have called your company twice. The first time, I was told by
the person answering the phone that she did not send out billing--call
Saturday and talk to Cerese. I called Cerese and she said she was
just putting on the bills information from the reports. She said I
would have to contact the Board of Directors to get the matter
straightened out.
As you can see, I am alarmed that you should send me a bill for
5225 for services that you supposedly rendered without our asking for
the services. In fact, I am very incensed about it.
THIS BILL WILL NOT BE PAID BY ME. I WILL BE FORWARDING I1 TO THE:
INSURANCE AGENCIES INVOLVED. HOWEVER. I AM ASKING THEM TO LOOK INTO
IT AND SEE IF THEY FEEL THAT IT IS A JUSTIFIED COST.
I do not agree with your following practises. (1 ) Your personell
do not identify themselves as being from the Tri-Area Ambulance
Service, a private company. (2)You do not state that a FEE will be
involved. (3)You were not called by us--do you expect me to pay for a
service not asked for--- (the Longmont Fire Protection District support
comes from my taxes; the Colo. St. Patrol comes from taxes) .
(4)People involved in accidents are not thinking as clearly as they
normally do--do you believe that you may be taking advantage of them
by asking them to sign "RELEASE" forms, and then billing them for a
service they were not aware that you were providing (or not providing
in my individual case) ?
My purpose is not to put you out of business(as I hear your
company is ailing) . But I believe some fairness should be involved.
And some knowledge of why you are operating in the mode you are bl
your personell that answer your phone.
I am sending a copy of this letter to appropriate agencies. I
hope a better method of operation will come of it.
