RE: APPROVAL OF AMENDED AGREEMENT BETWEEN WELD COUNTY AND HART-
FORD ACCIDENT AND INDEMNITY COMPANY AND AUTHORIZATION FOR
CHAIRMAN TO SIGN
WHEREAS , the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS , an agreement between Weld County and Hartford Acci-
dent and Indemnity Company became effective January 1 , 1983 , and
WHEREAS, it has been brought to the Board' s attention that
certain clauses concerning a 60 day notice prior to an anniversay
date and a 90 day cancellation notice were not contained in the
original agreement, and
WHEREAS, Hartford has submitted an amended agreement for the
Board' s approval, a copy of which is attached hereto and incorpor-
ated herein by reference, and
WHEREAS , the Board deems it advisable to approve said amended
agreement.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the amended agreement
between Weld County and Hartford Accident and Indemnity Company
be , and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be,
and hereby is , authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 25th day of
July, A.D. , 1983 .

RIDER
This Rider is attached to and made a part of the Individual Stop Loss
Agreement Effective January 1, 1983 between WELD COUNTY and HARTFORD ACCIDENT
AND INDEMNITY COMPANY.
It is understood and agreed that said Agreement is amended as follows:
Section III Paragraph a. as it appears on Page ISL-10 of said Agreement
is deleted replaced by the following:
a. Either you or we may terminate this Agreement as of any anniversary
date by giving the other party at least 60 days' advance written
notice of termination.
Section IV Paragraph b. as it appears on Page ISL-ll of said Agreement
is deleted and replaced by the following:
b. We may change the Monthly Premium Rate of Minimum Annual Premium
after giving you 90 days advance written notice of our intent to do
so. Such written notice will be mailed to your address as last
shown on our records and will supersede the amounts shown in the
Schedule and any prior such written notice(s) . However, we may not
make such a change sooner than the first anniversary date of this
Agreement.
This Rider is signed by each party below to take effect as of
January 1, 1983.
