Subject: Amendment to Transportation Agreement with Weld County Community
Center Foundation
Enclosed for Board approval is an amendment to the 1982 continuous agreement
with the Weld County Community Center Foundation to provide transportation
services to their clients.
The Community Center Foundation will provide reimbursement to Human Resources
at the rate of $.66 per mile for 121,212 miles of transportation. This is an
increase of $. 13 over the previous fiscal year. The reimbursement is used to
cover the costs of administrative overhead, gas and oil , repair, maintenance,
vehicle replacement and salaries.

PURCHASE OF SERVICES
AMENDMENT
THIS AMENDMENT, made and entered into this 5th day of November, 1984,
by and between the Weld County Division of Human Resources, hereinafter,
referred to as "Human Resources"; and the Weld County Comm nity Center Founda-
tion, hereinafter referred to as "WCCCF".
WITNESSETH
WHEREAS, Human Resources and WCCF are desirous of amending the original
Agreement of January 29, 1982.
NOW THEREFORE, the following conditions are mutually greed upon:
1. WCCCF agrees to reimburse Human Resources at $.66 per mile for
121,212 miles of transportation not to exceed $8 ,000, effective
for the WCCCF Fiscal Year. The reimbursement will be used to
cover the costs of administrative overhead, gas and oil, repair,
maintainance, vehicle replacement and salaries.
2. Human Resources and WCCCF agree to review quarte ly actual
transportation cost, reimbursement rates and total miles of
transportation, and make appropriate adjustments.
3. All terns and conditions of the original Agreeme t shall remain
in effect unless otherwise stated in this Amendm nt.
