KNOW ALL MEN BY THESE PRESENTS THAT:
The undersigned, Leroy Johnson, being of lawful age, being first ! ',
duly sworn on oath, deposes and says, to-wit:
1. That the Affiant is Vice President of Energy Oil, Inc., and
that the Affiant is authorized to make this Affidavit on
behalf of Energy Oil, Inc. and the remaining working interest
owners In and to the Oil and Gas Leases(s) described below.
2. That during the month of December. 1982 Energy Oil, Inc.,
on behalf of Itself and the remaining working interest owners
completed a commercial well identified as Energy Oil, Inc.-
-.Hankins 41. Llited in the Nis of Section 22
'township 5 North - Range 67 Welt. 601 £.M.---
Wald County, Colorado
which well was equipped for production and has been shut-in
from the date of completion in such manner as to extend the
primary term of the following described Oil and Gas Leases(s)
to-wit:
• Oil and Gas Lease dated March 24, 1970, recorded in Book 626
Reception No. 1547631 of the records of Weld County, Colot ido, !.
executed by and between The First National Bank of Greeley,
trustee under the Will if Adam D. Hankins, deceased,in favor
of William .1. Phelan.
Oil and Gas Lease dated March 24, 1970, recorded in Book 612
--- - Reception 41554466 of the records of Weld County, Colorado,
executed by and between Olive M.Leonard, Thelma E. Cowles,
Florence M. Beyer, Lynn Smith, and Uorthe Rankin in favor
of William J. Phelan. (Continued on attached page 2)
covering the following described lands situate la said County of Weld
State of Colorado, to-wit: !.
Township 5 North - Range 67 West, 6th P.M.
Section 22: E§, NWk (Unitized)
3. Affiant further states that this Affidavit is made and filed !
in compliance with the statutory requirements in regard thereto
(Title 38, Article 42, Section 106, Colorado Revised Statutes,
1973), so as to provide notice to the world of the extension of !,
said Oil and Gas Lease(s), as to the above described lands for !,
as long as such production continues.
