RESOLUTION
RE: APPROVAL OF ENDORSEMENT NO. V TO GROUP MASTER CONTRACT NO.
82-01760000 BETWEEN BLUE CROSS AND BLUE SHIELD OF COLORADO
AND WELD COUNTY EMPLOYEES AND AUTHORIZATION FOR CHAIRMAN
TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, Endorsement No. V to Group Master Contract No. 82-
01760000 between Blue Cross and Blue Shield and Weld County em-
ployees has been presented to the Board of County Commissioners
of Weld County, Colorado for approval, said Endorsement being
attached hereto and incorporated herein by reference, and
WHEREAS, said Endorsement No. V will be effective March 1,
1982, and
WHEREAS, the Board of County Commissioners has studied said
Endorsement and deems it advisable to approve the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that Endorsement No. V to
Group Master Contract No. 82-01760000 between Blue Cross and Blue
Shield of Colorado and Weld County employees be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be,
and hereby is, authorized to sign said Endorsement No. V.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 8th day of
September, A.D. , 1982, nunc pro tunc March 1, 1982.

Effective date of this endorsement: March 1 , 1982
The Contract identified above is hereby amended by this endorsement which is
issued to form a part of the Contract.
SCHEDULE C., "ENROLLMENT AND BENEFIT ELIGIBILITY", is amended as follows:
Section D., "Period of Continuous Employment Required for Eligibility", is
deleted and the following is substituted therefor:
D. Period of Continuous Employment Required for Eligibility. Employees are
eligible for enrollment under the Contract following completion of one full
pay period (from the 16th of the one month to the 15th of the next month) .
Section E., "Enrollment Requirements for Employees", Paragraph 1 ., is deleted
and the following is substituted therefor:
1 . Employees may enroll in membership under the Contract within one
month after the provisions of Section D. above are satisfied. Such coverage
under the Contract will become effective on the first day of the month
following one full pay period. The Employee must be on the payroll record on
the effective date of coverage. Coverage for Employees whose applications are
not received by Blue Cross and Blue Shield of Colorado within one month from
the end of one full pay period will be required to wait until the next regular
group reopening, unless otherwise specified below.
